OBJECTIF: La présente étude a été conçue pour évaluer la prévalence des complications de la grossesse chez une naissance vivante précédant l’apparition d’avortements récurrents.

MÉTHODES: Une étude cas-témoins comparant les femmes ayant eu au moins deux avortements spontanés consécutifs après une naissance vivante avec des témoins appariés, sans avortements récurrents, a été réalisée. Les cas ont été recrutés dans la clinique des avortements récurrents. Les femmes du groupe témoin ont été appariées par les paramètres suivants: âge, ordre de grossesse et naissance vivante la même année que le groupe étudié. Quatre contrôles ont été appariés pour chaque cas. L’analyse s’est concentrée sur les caractéristiques de la naissance vivante précédant les avortements récurrents du groupe d’étude et les naissances des témoins appariés.

RESULTATS: De janvier 2001 à décembre 2002, 140 femmes ont été examinées à la clinique ambulatoire pour des avortements récurrents. Le groupe à l’étude était composé de 58 femmes ayant eu une naissance vivante avant au moins deux avortements spontanés consécutifs, ce qui a été comparé à 232 témoins. Un taux plus élevé de prééclampsie (légère et sévère) statistiquement significatif a été observé chez une naissance vivante précédant des avortements récurrents par rapport aux témoins appariés (10,3% contre 3,9%, p = 0,047). En outre, une tendance non significative a été trouvée pour des taux plus élevés de motifs de fréquence cardiaque foetale non rassurants (8,6% par rapport à 3,0%, p = 0,055) dans ce groupe. Aucune autre différence significative concernant les complications maternelles ou néonatales telles que le décollement placentaire, la restriction de croissance intra-utérine et la mort du fœtus intra-utérin n’a été observée entre les groupes.

CONCLUSIONS: Une naissance vivante précédant l’apparition d’avortements récurrents est associée à un taux plus élevé de prééclampsie.

Abstract (English)  :

OBJECTIVE: The present study was designed to evaluate the prevalence of pregnancy complications in a live-birth preceding the appearance of recurrent abortions.

METHODS: A case-control study comparing women who had at least two consecutive spontaneous abortions after one live birth with matched controls, without recurrent abortions, was performed. Cases were recruited from the Recurrent Abortions Clinic. The women in the control group were matched by the following parameters: age, pregnancy order and having had a live birth in the same year as the study group. Four controls were matched for each case. The analysis focused on the characteristics of the live-birth preceding the recurrent abortions of the study group and the births of the matched controls.

RESULTS: From Jan 2001 through Dec 2002, 140 women were examined in the Outpatient Clinic for Recurrent Abortions. Of these, 58 women who had a live-birth prior to at least two consecutive spontaneous abortions comprised the study group, which was compared with 232 controls. A statistically significant higher rate of preeclampsia (mild and severe) was found in a live-birth preceding recurrent abortions than in the matched controls (10.3 vs. 3.9%, p=0.047). In addition, a nonsignificant trend was found for higher rates of non-reassuring fetal heart rate patterns (8.6 vs. 3.0%, p=0.055) in this group. No other significant differences regarding maternal or neonatal complications such as placental abruption, intrauterine growth restriction, and intrauterine fetal death were noted between the groups.

CONCLUSIONS: A live-birth preceding the appearance of recurrent abortions is associated with a higher rate of preeclampsia.
