OBJECTIFS: Le but de cette étude était de comparer les soins prénataux de routine entre neuf services européens de gynécologie obstétrique.

METHODOLOGIE DE L’ETUDE: Une enquête a été effectué entre octobre 1992 et novembre 1993 à l’aide d’un questionnaire. Celui-ci comprenait 118 questions recouvrant les examens cliniques et tests de dépistage réalisés systématiquement pendant la grossesse.

RESULTATS: L’enquête a révélé un certain nombre de différences entre les types de soins prénataux pratiqués dans les différents services. Ainsi on note des différences dans la pratique des tests sérologiques (toxoplasmose, SIDA et hépatite B) lors de la première visite, et dans la pratique du toucher vaginal, de l’échographie, ou de la cardiotocographie.

CONCLUSION: Les différences observées entre les soins prénataux reflètent seulement nos incertitudes concernant la validité des différents tests utilisés pour surveiller les grossesses. Alors que des restrictions financières nous imposent des choix, nous pensons qu’une évaluation de l’efficacité et du coût des soins prénataux est essentièle.

Abstract (English)  :

OBJECTIVE: The aim of this study was to compare routine prenatal care in nine European obstetrics and gynaecology departments.

STUDY DESIGN: A survey was performed between October 1992 and November 1993 by means of a questionnaire. The questionnaire comprised 118 questions covering clinical examinations and investigations systematically performed during the pregnancy.

RESULTS: The survey revealed a certain number of differences between the types of prenatal care performed by each department. Thus, one notices differences to do the serological testing (toxoplasmosis, AIDS and hepatitis B) at the first visit and in the practice of vaginal examination, systematic ultrasound examination or cardiotocography.

CONCLUSION: The differences observed in the prenatal care furnished are only a reflection of our uncertainties concerning the validity of the various tests used to monitor pregnancies. At a time when financial restrictions impose choices, we consider an evaluation of the efficacy and cost of prenatal care to be essential.

Les soins prénataux diffèrent d’un service à un autre en Europe (tests sérologiques, touchers vaginaux, échographie, etc…). Ces soins reflètent donc des croyances au lieu d’être basés sur des preuves.
