500 femmes ont participé à cette étude sur le stress psychologique associé à des interventions obstétricales et gynécologiques. Elles ont été recrutées par voie d’annonce dans des journaux locaux et nationaux, dans des magazines féminins et des de newsletters. Les femmes ont répondu à une enquête préliminaire sur leurs expériences des intervention obstétricales et gynécologiques, leur histoire personnelle, et leur sentiments sur l’intervention au moment où elle a eu lieu et au moment de d’enquête.

Sur 500 sujets, 100 femmes rapportent un historique d’intevention terrifiante ou particulièrement pénible (“terrifying“ or “very distressing“), “out of ordinary“ et qui date d’il y a plus d’un mois. Ces femmes ont reçu un questionnaire de suivi (PTSD-I) et 30 d’entre elles remplissent les critères DSM-III-R qui diagnostiquent les troubles de stress post-traumatique (PTSD post traumatic stress disorders).

Des différences significatives apparaissent entre les 30 femmes avec PTSD et 30 autres qui ont évalué leur expérience de “très bonne“ à “un peu pénible“. Parmi ces différences on trouve: sentiment d’impuissance pendant l’intervention, manque d’information donnée à la patiente, expérience de douleur physique, perception d’une attitude antipathique [“on the part of examiner“], et manque de consentement pleinement compris par le patient pour l’intervention (“lack of clearly-understood consent by the patient for the procedure“).

Ces résultats sont mis en relation avec les données de la littérature sur les réactions émotionnelles des femmes lors de l’accouchement, sur la violence sexuelle et sur d’autres causes de stress post-traumatique. Nos résultats suggèrent une cause de stress post-traumatique non décrite précédemment, et appelle à une remise en question des pratiques médicales courantes dans le domaine de l’obstétrique et de la gynécologie

Abstract (English)  :

500 women volunteers took part in a study about the psychological stress associated with obstetric and gynecological procedures. The sample was recruited by advertisements in local and national newspapers and in women’s magazines and newsletters.

Women completed a preliminary questionnaire on their experiences of obstetric and gynecological procedures, their biographical data, and their feelings associated with the procedures both at the time and now. Out of the 500 subjects, over 100 women gave an history of an obstetric and/or gynaecological which was ’very distressing’ or ’terrifying’, which was ’out of the ordinary’ and which occurred ’more than one month previously’. These women were sent follow-up PTSD-I questionnaires and 30 of them fulfilled the DSM-III-R criteria for a diagnosis of post-traumatic stress disorder (PTSD).

Significant differences between the 30 women with PTSD, and 30 respondents who rated their experiences from ’very good’ to ’slightly distressing’, were found on a range of findings, including feelings of powerlessness during the procedures, lack of information given to the patient, the experience of physical pain, a perceived unsympathetic attitude on the part of the examiner, and a lack of clearly-understood consent by the patient for the procedure.

The results are discussed in relation to the literature on women’s emotional reactions to the childbirth process and also to sexual violence and other causes of PTSD. The present findings suggest a cause of PTSD not previously described and challenge current medical working practices in the area of obstetrics and gynaecology.
