OBJECTIF: Le but de cette étude était de comparer les stratégies d’administration de corticostéroïdes pour la prévention de la morbidité et de la mortalité néonatales.

DESIGN : Un modèle décisionnel de Markov a comparé 3 stratégies d’administration de corticostéroïde antepartum: (1) à toutes les femmes enceintes (universel), (2) à toutes les femmes enceintes ayant déjà accouché avant terme (risque élevé), et (3) aux femmes ayant des symptômes de travail prématuré qui les exposaient à un risque d’accouchement dans les 7 jours (courant). Un deuxième modèle avec l’ajout d’un bras de « sauvetage » pour aider les femmes qui n’ont pas réussi à accoucher a également été créé.

RÉSULTATS : Comparée à la stratégie actuelle, la stratégie universelle permettrait de réduire d’environ 1 000 le nombre de cas de syndrome de détresse respiratoire et de créer plus de 3 millions de femmes supplémentaires par an. L’ajout d’un bras de sauvetage réduit davantage les taux de morbidité et de mortalité.

CONCLUSION : Une stratégie universelle d’administration de corticostéroïdes confère un avantage potentiel pour la prévention de la morbidité néonatale ou du décès par rapport à la stratégie actuelle, mais nécessite le traitement d’un grand nombre de femmes.

Abstract (English)  :

OBJECTIVE: The purpose of this study was to compare strategies of corticosteroid administration for the prevention of neonatal morbidity and death.

STUDY DESIGN: A Markov decision model compared 3 strategies of antepartum corticosteroid administration: (1) to all pregnant women (universal), (2) to all pregnant women with a previous preterm delivery (high risk), and (3) to women who had preterm labor symptoms that placed them at risk for delivery within 7 days (current). A second model with addition of a “rescue“ arm to capture women who remained undelivered was also created.

RESULTS: Compared with the current strategy, the universal strategy would result in roughly 1000 fewer cases of respiratory distress syndrome and > 3 million more women would receive corticosteroids annually. The addition of a rescue arm further reduces morbidity and mortality rates.

CONCLUSION: A universal strategy of corticosteroid administration confers potential benefit for the prevention neonatal morbidity or death over the current strategy but requires that a large number of women be treated.
