AIM: The term ’postnatal depression’ conveys the notion of a homogenous disorder, which can be investigated and treated as if it had a single cause. This paper argues that adverse events during childbirth can act as powerful triggers in the onset of posttraumatic stress disorder (PTSD) and subsequently, postnatal depression. Drawing on the qualitative data gathered during a mixed method, longitudinal prospective study with childbearing women (n = 499), three case studies are used to illustrate trauma responses to stressful childbirth experiences.
RESULTS: Childbirth events involving life threat and personal injury were found to give rise to trauma reactions. Responses included the re-experiencing of the trauma commonly through intrusive thoughts about the birth, avoidance of people. places and thoughts associated with the event, and physiological arousal symptoms that may contribute to sleep difficulties or somatic complaints. Furthermore, the women reported the early onset of trauma symptoms, avoiding contact with the baby, and the experience of depression.
CONCLUSION: Trauma symptoms in the postpartum period may be missed and their relationship to maternal depression may be overlooked. Health professionals need to acknowledge the unique nature of childbirth for each woman and attend to the emotional aspects of care in order to minimize adverse psychosocial consequences.
