Le choix du lieu de naissance par une femme enceinte a-t-il une influence sur la façon dont elle perçoit les interventions obstétricales? Ce choix a-t-il une influence sur la fréquence des interventions obstétricales lors de l’accouchement? (hollande). Birth.2004.

SITUATION: Aux Pays-Bas, les sages-femmes exerçant à domicile font partie intégrante du système de soins périnataux. On a émis l’hypothèse que l’accouchement à domicile a une influence positive sur le déroulement de la naissance. Ce serait la raison pour laquelle le nombre d’interventions obstétricales aux Pays-Bas est plus bas que dans les pays voisins. Cette étude a analysé l’impact du choix du lieu de naissance (à domicile ou à l’hôpital) sur le déroulement de la grossesse et de l’accouchement régulièrement suivis par une sage-femme.

METHODE: Il s’agit d’une étude prospective de 624 femmes enceintes présentant des grossesses à bas risque, enceintes de 20 a 24 semaines. Ces femmes ont été suivies par des sages-femmes appartenant a 25 “independent working midwifery practices” (cabinet médicaux tenus par des sages-femmes). L’évaluation du déroulement de l’accouchement s’est faite sur la base du nombre d’interventions effectuées par les sages-femmes et les obstétriciens.

RESULTATS: Les femmes ayant choisi d’accoucher à la maison ont bénéficié d’une approche moins technologique par rapport aux femmes ayant choisi d’accoucher à l’hôpital. Les données ont établi un lien entre le choix du lieu de naissance et le nombre d’interventions: les femmes ayant choisi l’accouchement à la maison ont plus de chance d’avoir une rupture artificielle des membranes ou une amniotomie pratiquées par les sages-femmes. Les multipares qui avaient choisi d’accoucher à l’hôpital ont eu des consultations plus fréquentes et ont plus souvent été redirigées vers un obstétricien. Dans le groupe de femmes multipares qui ont été redirigées vers un obstétricien, celles dont le choix du lieu de naissance était à l’origine le domicile ont subi moins d’interventions médicales (déclenchement, accélération du travail, interventions pharmacologiques pour soulager la douleur, extractions instrumentales, césariennes) par rapport aux femmes qui avaient choisi en premier lieu d’accoucher a l’hôpital.

CONCLUSIONS: Un grand nombre de femmes souhaitent accoucher à la maison. On a pris comme critère d’évaluation du déroulement de l’accouchement le nombre d’interventions obstétricales effectuées. Un faible nombre d’interventions correspondait à un déroulement harmonieux de l’accouchement. Nous pensons que des facteurs psychologiques (attentes et perceptions de la mère) sont à l’origine du choix du lieu de la naissance et influencent la façon dont se déroulera l’accouchement.

Abstract (English)  :

BACKGROUND: A home confinement with midwifery care is still an integral part of Dutch maternity care. It has been argued that the existence of home birth itself influences the course of the birth process positively, which is why obstetric interventions are low in comparison with neighboring countries. This study examined the impact of women’s intended place of birth (home or hospital) and the course of pregnancy and labor when attended by midwives.

METHODS: This is a prospective study of 625 low-risk pregnant women, gestation 20 to 24 weeks, enrolled in 25 independently working midwifery practices. The course of labor was measured by the frequency of interventions by midwives and obstetricians.

RESULTS: A more non-technological approach to childbirth was observed within the women opting for a home birth compared with the women opting for a hospital birth. Data showed a relationship between interventions and planned birth site: sweeping membranes and amniotomy by midwives were more likely to be conducted in women opting for a home birth. Multiparas opting for hospital birth were more likely to experience consultations and referrals. Within the group of multiparas referred for obstetrician care, women intending to have a home birth experienced fewer interventions (e.g., induction, augmentation, pharmacologic pain relief, assisted delivery, cesarean section) compared with those who had opted for a hospital birth.

CONCLUSIONS: A large proportion of women desire a home birth. The impact of that choice demonstrated a smoother course of the birth process, compared with women who desired to deliver in the hospital, as measured by fewer obstetric interventions. We suggest that psychological factors (expectation and perceptions) influence both a woman’s decision of birthplace and the actual birth process.
