OBJECTIFS : Décrire les résultats de naissances à domicile planifiées dans l’exercice de sages-femmes diplômées (certified nurse-midwives).
METHODES : 29 sages-femmes américaines a été recrutées dans cette étude en 1994. Les femmes se présentant pour une naissance à domicile ont participé à cette étude de fin 1994 à fin 1995. Les résultats ont été recueillis pour la totalité des femmes qui ont participé. La validité et la fiabilité des données ont été établies.
RESULTATS : Parmi les 1404 femmes qui ont participé à l’étude et qui avaient l’intention d’accoucher à domicile, 6 ont fait une fausse couche, ou leur grossesse a été interrompue, ou ont changé d’avis. 7,4 % n’étaient plus éligibles pour une naissance à domicile en raison de problèmes périnataux avant le début du travail à terme et ont été transférées en hôpital. Parmi les femmes dont le travail a débuté avec l’intention de donner naissance à domicile, 102 (8,3%) ont été transférées à l’hôpital durant le travail. 10 (0,8 %) mères ont été transférées à l’hôpital après l’accouchement et 14 enfants ont été transférés après leur naissance. Le total des mortalités intra partum et néonatales pour les femmes dont le travail a débuté avec l’intention d’accoucher à domicile était de 2,5 pour 1000. Pour les femmes qui ont effectivement accouché à domicile, la mortalité intra partum et néonatale était de 1,8 pour 1000.
CONCLUSION : Les naissances à domiciles peuvent se dérouler avec de bons résultats sous la surveillance de professionnels qualifiés dans un système qui facilite le transfert à l’hôpital lorsque nécessaire. La mortalité intra partum lors des naissances prévues à domicile est essentiellement le fait de naissances post terme avec preuve de passage de méconium.

Abstract (English)  :

OBJECTIVE: To describe the outcomes of intended home birth in the practices of certified nurse-midwives.
METHODS: Twenty-nine US nurse-midwifery practices were recruited for the study in 1994. Women presenting for intended home birth in these practices were enrolled in the study from late 1994 to late 1995. Outcomes for all enrolled women were ascertained. Validity and reliability of submitted data were established.
RESULTS: Of 1404 enrolled women intending home births, 6% miscarried, terminated the pregnancy or changed plans. Another 7.4% became ineligible for home birth prior to the onset of labor at term due to the development of perinatal problems and were referred for planned hospital birth. Of those women beginning labor with the intention of delivering at home, 102 (8.3%) were transferred to the hospital during labor. Ten mothers (0.8%) were transferred to the hospital after delivery, and 14 infants (1.1%) were transferred after birth. Overall intrapartal fetal and neonatal mortality for women beginning labor with the intention of delivering at home was 2.5 per 1000. For women actually delivering at home, intrapartal fetal and neonatal mortality was 1.8 per 1000.
CONCLUSION: Home birth can be accomplished with good outcomes under the care of qualified practitioners and within a system that facilitates transfer to hospital care when necessary. Intrapartal mortality during intended home birth is concentrated in postdates pregnancies with evidence of meconium passage.
