Ojectifs: Déterminer l’incidence de la naissance à domicile en Nouvelle-Zélande entre 1973 et 1993, au niveau national et régional (uniquement en 1991-1992). On classe sous la définition d’accouchement à domicile les accouchements pour lesquels le lieu de naissance choisi était, au moment du début du travail, le domicile. Il s’agit de calculer les taux de mortalité maternelle et périnatale pour les naissances à domicile et de catégoriser les causes de la mortalité périnatale.

Méthodes: On a analysé les données (data sheet) de 9776 naissance prévues à domicile. Ces données (data sheet) provenaient d’Aotearoa, l’organisation nationale des associations pour la naissance a domicile de Nouvelle-Zélande. Ces données ont été comparées aux chiffres de périnatalité nationaux et aux chiffres de périnatalité du National Woman’s Hospital d’Auckland. L’analyse a été effectuée par régression de Poisson avec prise en compte de la sur-dispersion (trend analysis was performed by Poisson regression allowing for overdispersion)

RESULTATS: Les accouchements planifiés à domicile ont représenté 2% des naissances totales en 1993, en progression de 0,04% depuis 1973.Le taux de mortalité périnatale des naissances à domicile sur cette période était de 2,97 pour 1000 naissances, sans évolution au cours du temps. Ce taux n’était pas significativement différent du taux de mortalité périnatale dans un groupe sélectionné d’accouchements à bas risque au National Women’s Hospital. 31% des décès périnatals étaient dus à des anomalies létales. Il y a eu un décès maternel (taux de mortalité maternel : 1,02 pour 10 000 naissances au total). Il y a eu des différences significatives des taux d’accouchement à domicile selon les régions (area health board region) en 1991-1992.

Conclusion
En Nouvelle-Zélande entre 1973 et 1993, l’accouchement à domicile était un choix sûr et en augmentation, même s’il reste minoritaire.

Abstract (English)  :

AIMS: To determine for the period 1973-93, national and regional (1991 and 1992 only) incidence of home birth in New Zealand, with home birth defined as home being the intended place of birth at the onset of labour, to calculate perinatal and maternal mortality rates for home birth, and to categorise the cause of perinatal death.

METHODS: Data sheets for 9776 planned home births were analysed. These had been collected by the Home Birth Associations of New Zealand/Aotearoa. National perinatal data and data from National Women’s Hospital, Auckland were used for comparison. Trend analysis was performed by Poisson regression allowing for overdispersion.

RESULTS: Planned home birth made up 2% of the total births in 1993, up from 0.04% in 1973. The home birth perinatal mortality rate for this period was 2.97 per 1000 total births, with no change over time. This was not significantly different from the rate for a selected low risk group at National Women’s Hospital. Lethal anomalies caused 31% of the perinatal deaths. There was one maternal death (maternal mortality rate: 1.02 per 10,000 total births). There were significant differences in the rate of home birth in separate area health board regions for 1991 and 1992.

CONCLUSION: Home birth was a safe and increasingly popular: though minor, option for New Zealand women from 1973-93.
