L’objet de cette étude était d’observer les différences entre les femmes qui accouchent avec ou sans péridurale. Cette étude est en partie la réplique d’une autre, par Poore et Foster [(1985) Epidural and no epidural anaesthesia: differences between mothers and their experience of birth, Birth, 12(4), 205-212] qui s’intéressait aux différences entre les mères vis à vis de leur choix de péridurale. Quarante-six femmes ont été suivies, sur six mois, toutes ayant accouché vaginalement. On a évalué pour chacune d’elles la peur de l’accouchement, le désir de participer au processus de l’accouchement, et la connaissance des risques associés à la péridurale.

Les femmes qui ont choisi la péridurale exprimaient une peur importante de l’accouchement, un besoin de contrôle de l’accouchement en provenance de l’extérieur, et le souhait de s’en remettre passivement au processus de la naissance. Les résultats ont aussi montré que les femmes qui accouchaient sans péridurale avaient moins peur de l’accouchement, un besoin de contrôle interne, et un souhait de participer activement dans le processus.

Les femmes qui ont choisi, avant leur accouchement, de ne pas avoir recours à la péridurale, avaient un meilleur score sur une échelle destinée à évaluer leur connaissance des risques associés à la péridurale.

Ces résultats montrent que les différences dans les croyances à propos de la naissance sont corrélées avec les choix de contrôle de la douleur.

Abstract (English)  :

The purpose of this study was to look at the differences between women who labour with or without epidural anaesthesia. This study was a partial replication of a study done by Poore and Foster [(1985) Epidural and no epidural anaesthesia: differences between mothers and their experience of birth, Birth, 12(4), 205-212] looking at differences in mothers and the epidural choice. Forty∆six women were surveyed within 6 months of vaginal delivery. Fear of childbirth, locus of control for childbirth, desired participation in the childbirth process, and knowledge of epidural risk were assessed. Women who chose to deliver with an epidural had high fear of childbirth, an external locus of control for childbirth, and a desire for passive compliance in the childbirth process. Results also showed that women who laboured without an epidural had low fear of childbirth, an internal locus of control for childbirth, and a desire to actively participate in the childbirth process. Women who decided not to have an epidural prior to going into labour scored higher on a scale designed to assess knowledge of risks associated with epidural use. These results reveal that differences in beliefs about childbirth are related to pain control choices.
