OBJECTIVE: To investigate whether the length of the interval between an abortion and the next pregnancy is associated with increased risks of adverse maternal and perinatal outcomes in Latin America.

METHOD: Retrospective cross-sectional study using information from 258,108 women delivering singleton infants and whose previous pregnancy resulted in abortion recorded in the Perinatal Information System database of the Latin American Centre for Perinatology and Human Development, Montevideo, Uruguay, between 1985 and 2002. Adjusted odds ratios were obtained through logistic regression analysis.

RESULT: Compared with the post-abortion interpregnancy intervals of 18 to 23 months, intervals shorter than 6 months were significantly associated with increased risks of maternal anemia, premature rupture of membranes, low birth weight, very low birth weight, preterm delivery, and very preterm delivery.

CONCLUSION: In Latin America, post-abortion interpregnancy intervals shorter than 6 months are independently associated with increased risks of adverse maternal and perinatal outcomes in the next pregnancy.

DEFINITION: Post-abortion interpregnancy interval (PAII): the time elapsed between the day of the abortion and the first day of the last menstrual period for the index pregnancy.

Grossesse : un intervalle de 18 mois à 5 ans semble être nécessaire

Avant même la naissance de la « médecine scientifique » on se plongeait en
conjectures sur les facteurs pouvant influencer, voire augmenter, les
risques liés aux problèmes des naissances… des êtres humains. Aujourd’hui les
connaissances ont considérablement évolué dans ce domaine mais quelques
zones d’ombre subsistent en ce qui concerne les causes de la mortalité
périnatale. Une méta-analyse réalisée par des chercheurs Sud-Américains a
montré que les femmes pourraient améliorer l’état de santé de leurs
enfants – voire réduire la morbi-mortalité - en respectant un intervalle de
dix huit mois à cinq ans maximum entre deux grossesses. Les résultats de
leurs travaux viennent d’être publiés dans le Journal of the American
Association of Medicine (JAMA) après l’analyse de 67 études internationales
sur la question.

L’étude, coordonnée par le Dr Agustin Conde-Agudelo du Centro de Estudios e
Investigación en Salud de la Fundación Santa Fe de Bogotá en Colombie, a
passée en revue 130 articles des travaux publiés entre 1996 et janvier 2006
disponibles sur différentes bases de données (MEDLINE, EMBASE, ECLA,
POPLINE, CINAHL, et LILACS) dont 67 (52%) ont été retenus. Selon les
chercheurs, à en croire ces travaux - qui portent sur plus de 11 millions de
grossesses, des intervalles trop rapprochées ou au contraire trop espacées
augmentent le risque de complications chez les nouveau-nés, notamment de
naissance prématurée ou de petit poids de naissance.

Selon Conde-Agudelo et coll., « entre deux grossesses, pour chaque mois de
moins que les 18 mois requis, le risque d’avoir un bébé prématuré augmente
de 1,9%. Au-delà d’une période de 59 mois, les chances de mort prématurée
progressent de 0,6% par mois supplémentaire ». D’après les résultats de l’
étude, les risques liés à des intervalles trop courts, ou au contraire trop
longs, sont valables pour les pays riches comme pour les pays pauvres. En
témoignent des études menées aux Etats-Unis, en Europe, en Afrique et en
Amérique Latine.

Il faut souligner que, dans cette étude, les chercheurs restent au niveau du
constat. Quant à savoir pourquoi un intervalle trop long entre deux
grossesses n’est pas sans risques, ils avancent comme hypothèse que le temps
peut diminuer l’aptitude d’une femme à procréer et que certains facteurs
diminuant la fertilité peuvent aussi entraîner un mauvais développement du
foetus.

En effet, la grossesse et l’allaitement nécessitent des nutriments provenant
de l’organisme de la femme, rappellent les auteurs de l’étude. « Et pour une
femme, être enceinte une nouvelle fois avant que les réserves ne soient
renouvelées signifie plus de risques pour son bébé ».
