L’introduction d’analogues de prostaglandines et de mifépristone a modifié la gestion de l’avortement du deuxième trimestre au cours des deux dernières décennies. Le gemeprost et le misoprostol sont les deux analogues de la prostaglandine les plus étudiés et utilisés au cours de cette période. La combinaison du géméprost ou du misoprostol avec la mifépristone est la plus efficace. Avec ces méthodes, plus de 90% des femmes avortent dans les 24 heures et l’intervalle moyen entre le déclenchement et l’avortement est d’environ 6 heures. La mifépristone est chère et n’est pas disponible dans de nombreux pays. Par conséquent, les schémas thérapeutiques utilisant uniquement des analogues de prostaglandines pourraient constituer la seule option. Ces schémas thérapeutiques sont toujours efficaces avec un taux d’avortement supérieur à 90% en 48 heures. Cependant, l’intervalle entre le déclenchement et l’avortement (15 heures) est beaucoup plus long. Les tentes intra-cervicales peuvent être utilisées pour raccourcir le déclenchement aux intervalles d’avortement.

Abstract (English)  :

The introduction of prostaglandin analogues and mifepristone has changed the management of second trimester abortion in the last 2 decades. Gemeprost and misoprostol are the two most extensively studied prostaglandin analogues that are used in this period. The combination of either gemeprost or misoprostol with mifepristone is most effective. With these regimens, over 90% of women abort within 24 hours and the mean induction to abortion interval is about 6 hours. Mifepristone is expensive and is not available in many countries. Therefore, prostaglandin analogue-only regimens might be the only option. These regimens are still effective with an abortion rate of >90% in 48 hours. However, the induction to abortion interval (15 hours) is much longer. Intra-cervical tents can be used to shorten the induction to abortion intervals.
