Abstract (English)  :

The aim of the study was to develop an instrument to measure midwifery care in relation to World Health Organization’s classification of care in normal birth and to test the instrument for content validity and inter-rater reliability.

Methods. The Delphi method was used for development of the instrument and to elicit evidence of content validity. Six experts from three different geographical regions in Sweden, representing clinically working midwives, lecturers in midwifery and obstetricians, participated in the first part of the study. The instrument was tested for inter-rater reliability in an exploratory study by two midwives and one of the authors. Data were analysed using percentage of agreement level and the Kappa coefficient.

Results. Five expert rounds were needed to reach consensus for content validity. The inter-rater reliability test showed high agreement levels (95.9, 94.2 and 95.7%) and good to very good Kappa coefficients (0.74-1.0). The final instrument consisted of 78 items divided into five sections: background (five items); practices which are demonstrably useful and should be encouraged (55 items); practices which are clearly harmful or ineffective and should be eliminated (five items); practices for which insufficient evidence exists to support a clear recommendation and which should be used with caution while further research clarifies the issue (four items); and finally practices which are frequently used inappropriately (nine items).

Conclusions and relevance to clinical practice. The instrument can be used at a labour ward to measure documented care and quality of midwifery care. The results can be used to identify areas for improvements, to develop guidelines towards evidence-based care and to improve documentation. However, the present study should be regarded as an exploratory study and the feasibility of the instrument remains to be tested in empirical studies.

Il est superflu de se concentrer autant sur la technique durant les accouchements ordinaires. Une partie du traitement est pratiqué seulement par habitude, sans que cela soit vraiment un besoin, montre une nouvelle étude.

Les accouchements ressemblaient plus à des soins en unité intensive qu’un endroit oú l’on peut mettre au monde un enfant dans le calme et la tranquilité et donc je me suis demandé si cela était vraiment des soins basés sur la medecine factuelle, dit Fia Sandin Bojö, sage-femme.

Elle décide alors d’étudier comment la maternité de l’Hôpital Central de Karlstad s’accorde avec les recommandations basée sur la médecine factuelle que donne l’OMS, conseils qui sont construits sur des recherches bien documentées.

Et cela montrait que peu de différences étaient faites entre les femmes classées accouchement attendu normal et celle classées accouchement attendu compliqué. Les femmes recevaient des gouttes pour stimuler les contractions et des lavements sans que cela soit nécessaire. Et les enfants avaient les bruits fœtaux surveillés trop souvent. Trop souvent du fait que l’équipement qui est employé est si sensible qu’il alarme souvent inutilement. Et cela peut donc, entraîner à utiliser inutilement la ventouse ou même la césarienne.

Fia Sandin Bojö a fait modifier les routines pour l’accouchement á Karlstad. Avec de bons résultats dit elle :
- Il y a autant de mamans et d’enfants qui vont bien et davantage d’enfants qui vont mieux, après que l’on ait donner davantage le temps au temps, qu’on ait plus de patience. Que l’on soit moins rapide á mettre en place des perfusions et autre. Tout c’est amélioré , dit Fia Sandin Bojö.

Maintenant Fia Sandin Bojö pense que les conseils du Bureau Social (1) vieux d’il y a six ans, qui servent de base pour les maternités d’aujourd’hui, doivent être révisés. Modifier les recommandations offielles est - entre autres - de la responsabilité du syndicat des sages-femmes, et sa présidente Anna Nordfield reprend á son compte plusieurs remarques de Fia Sandin Böjo. Une révision des conseils de bonne pratique est en cours dit-elle.

Nous avons eu entre autres une discussion sur nos groupes de consensus scientifique, sur comment nous pourrions travailler avec une mise à jour. L’ensemble des connaissances change tout le temps, et de temps en temps l’on doit s’arrêter et vérifier si nous travaillons réellement de la meilleure maniére, dit Anna Nordfjell.
