Objectif. Notre objectif était de déterminer les conséquences maternelles et fœtales de la position latérale pour l’accouchement.
Population et méthode. Nous avons effectué une étude cas/témoin rétrospective comparant les patientes ayant accouché en décubitus latéral (DL) à la Maternité Régionale de Nancy entre août 2004 et août 2005, à des patientes ayant accouché en position gynécologique aménagée (APOR B. de Gasquet®).

Résultats. Dans les deux groupes, la durée du travail était quasi semblable, avec une légère augmentation des efforts expulsifs dans le groupe DL. L’orientation de dégagement est significativement différente (p = 0,003) avec 72 % d’orientations obliques en DL. Il n’y avait pas de différence significative pour l’adaptation des nouveau-nés.

En DL, on retrouve significativement plus de périnées intacts ou éraillés (50 % versus 25 %, p = 0,03) ainsi qu’une diminution non significative des épisiotomies (6 % versus 18 %). Aucune hémorragie de plus de 500 ml n’a été constatée. Toutes les parturientes ont été satisfaites par leur position d’accouchement latérale.

Conclusion. Aucune conséquence lors de la pratique de l’accouchement sur le côté n’a été constatée, avec même un effet protecteur pour le périnée des femmes.

Abstract (English)  :

Delivery in the lateral decubitus position, an option for parturients in a level III maternity in Nancy.
Objective. We wanted to learn more about the maternal and fetal consequences of deliver in the lateral decubitus position.
Study population. This retrospective case-control study included women who delivered in the lateral decubitus position at the Nancy Regional Maternity ward from August 2004 to August 2005 in comparison with women who delivered in the gynecological position (APOR B. de Gasquet®).

Results. Duration of labor was similar in the two groups with a slightly greater degree of expulsion efforts in the lateral decubitus group. Orientation was slightly different (p=0.003) with 72% oblique orientations in the lateral decubitus group. The rate of intact perineum was also greater in the lateral decubitus group (50% vs 25%, p=0.03) with a lower rate of episitomy (6% vs 18%). There were no cases of hemorrhage greater than 500 ml. All parturients were satisfied with the lateral decubitus position during delivery.
