L’objectif général de l’étude était d’évaluer l’impact de la formation du personnel d’une maternité française de niveau 2a à l’accouchement physiologique, d’une part sur les paramètres obstétricaux et d’autre part sur le vécu des patientes. Cette évaluation d’une intervention de santé publique a été faite par une étude de type avant/après portant sur deux échantillons incluant respectivement 123 puis 153 parturientes. Les recueils des données étaient séparés par une période de quatre mois durant lesquels eut lieu la formation spécifique.

Les deux populations étaient comparables. Nous avons objectivé dans le second échantillon une diminution significative des rythmes cardiaques fœtaux pathologiques, une amélioration du statut périnéal à l’accouchement (moins d’épisiotomies, moins de déchirures), sans variation significative de l’adaptation cardio-respiratoire néonatale. Le personnel soignant et les parturientes ont bien adhéré au respect de la physiologie durant la grossesse et le travail. Le confort de la parturiente durant le travail était significativement amélioré.

Abstract (English)  :

Impact of training in physiological delivery on maternal land perinatal outcome (a before/after survey in a type 2A maternity)

The purpose of this study was to evaluate the impact of a training method for accompanying labour and birth according to physiologic considerations and was undertaken in the French maternity of Voiron. This study was performed with the so-called before/after intervention methodology; two samples of 123 and 153 parturients were compared (obstetrical and neonatal data) in order to determine the impact of the caregiver’s formation taking place between the periods where these two samples were randomly selected.

The two samples were identical for social and demographic data. There was a significative reduction of 50% of the anomalies of fetal cardiac monitoring and a strong tendency (p=0.10) in reduction of perineal lacerations or incisions after the intervention and finally a self-reported questionnaire was fulfilled by the parturients and indicated that there was a better well-being during parturition among the women of the second sample.
