OBJECTIF : Quantifier les facteurs de risque et les facteurs de risque d’accouchement par césarienne associés au déclenchement médical et au déclenchement électif du travail chez les femmes nullipares.

MÉTHODES : Une étude de cohorte prospective a été réalisée chez des femmes nullipares au terme d’une gestation vertex singleton ayant eu un travail induit dans 2 centres d’obstétrique. Les indications médicales et électives et les scores de Bishop ont été enregistrés avant le déclenchement du travail. Les données obstétriques et néonatales ont été analysées et comparées aux résultats obtenus chez les femmes accouchant spontanément. Les données ont été analysées à l’aide de modèles de régression univariés et multivariés.

RÉSULTATS : Un total de 1 389 femmes ont été incluses dans l’étude. Le taux d’accouchement par césarienne était de 12,0% chez les femmes ayant un début de travail spontané (n = 765), 23,4% chez les femmes en phase de déclenchement du travail pour des raisons médicales (n = 435) (rapport de cotes non ajusté [OU] 2,24 ; intervalle de confiance de 95% [IC] 1,64–3,06) et 23,8% chez les femmes dont le travail était induit électriquement (n = 189) (OR non ajusté 2,29 ; IC 95% 1,53–3,41). Cependant, après ajustement pour le score de Bishop à l’admission, aucune différence significative dans les taux d’accouchement par césarienne n’a été trouvée parmi les 3 groupes. Un score de Bishop de 5 ou moins était un facteur de risque prédominant pour une naissance par césarienne dans les 3 groupes (OR ajusté 2,32 ; IC 95% 1,66–3,25). Parmi les autres variables présentant un risque significativement accru d’accouchement par césarienne, on peut citer l’âge maternel de 30 ans ou plus, l’indice de masse corporelle de 31 ans ou plus, l’utilisation de l’analgésie épidurale au cours du premier stade du travail et le poids à la naissance de 3 500 g ou plus. Dans les deux groupes d’induction, davantage de nouveau-nés ont besoin de soins néonatals, davantage de mères ont besoin d’une transfusion sanguine et le séjour à l’hôpital maternel a été plus long.

CONCLUSION : Par rapport au début spontané du travail, le déclenchement médical et choisi du travail chez les femmes nullipares à terme avec un seul fœtus dans la présentation céphalique est associée à un risque accru d’accouchement par césarienne, principalement lié à un score défavorable de Bishop à l’admission.

Abstract (English)  :

OBJECTIVE: To quantify the risk and risk factors for cesarean delivery associated with medical and elective induction of labor in nulliparous women.

METHODS: A prospective cohort study was performed in nulliparous women at term with vertex singleton gestations who had labor induced at 2 obstetrical centers. Medical and elective indications and Bishop scores were recorded before labor induction. Obstetric and neonatal data were analyzed and compared with the results in women with a spontaneous onset of labor. Data were analyzed using univariate and multivariable regression modeling.

RESULTS: A total of 1,389 women were included in the study. The cesarean delivery rate was 12.0% in women with a spontaneous onset of labor (n = 765), 23.4% in women undergoing labor induction for medical reasons (n = 435) (unadjusted odds ratio [OR] 2.24; 95% confidence interval [CI] 1.64–3.06), and 23.8% in women whose labor was electively induced (n = 189) (unadjusted OR 2.29; 95% CI 1.53–3.41). However, after adjusting for the Bishop score at admission, no significant differences in cesarean delivery rates were found among the 3 groups. A Bishop score of 5 or less was a predominant risk factor for a cesarean delivery in all 3 groups (adjusted OR 2.32; 95% CI 1.66–3.25). Other variables with significantly increased risk for cesarean delivery included maternal age of 30 years or older, body mass index of 31 or higher, use of epidural analgesia during the first stage of labor, and birth weight of 3,500 g or higher. In both induction groups, more newborns required neonatal care, more mothers needed a blood transfusion, and the maternal hospital stay was longer.

CONCLUSION: Compared with spontaneous onset of labor, medical and elective induction of labor in nulliparous women at term with a single fetus in cephalic presentation is associated with an increased risk of cesarean delivery, predominantly related to an unfavorable Bishop score at admission.
