Traditionnellement, la grossesse était considérée comme « post-terme » à 42 semaines complètes de gestation. À cette gestation, si le col de l’utérus est défavorable, la meilleure pratique a été débattue entre le déclenchement systématique du travail et la gestion des patientes avec une forme de surveillance fœtale sérielle. La sagesse populaire semble être que la méta-analyse des essais contrôlés randomisés disponibles a réglé la question en faveur du déclenchement de routine. Le plus grand essai inclus, contenant plus de la moitié des cas (n = 3407), a été mené au Canada et publié en 1992. Les résultats de la méta-analyse ont conduit la Société des obstétriciens et gynécologues du Canada (SOGC) à publier des guides de pratique clinique en 1997.

Les lignes directrices recommandaient ce qui suit :

1. Après 41 semaines de gestation, si les dates sont exactes, les femmes devraient se voir proposer un accouchement volontaire ;

2. si le col est défavorable, il faut entreprendre la maturation ;

et 3. si la gestion en attente est choisie, une évaluation de la santé fœtale doit être initiée.

Il est présumé que les essais contrôlés randomisés ou, mieux encore, les méta-analyses d’essais randomisés, constituent la meilleure preuve pour déterminer les soins appropriés. Cependant, une fois que les informations ont été déclarées « les meilleures preuves disponibles », en particulier si cette affirmation est utilisée pour justifier des directives de pratique clinique ou un « consensus », toute enquête plus poussée peut être inhibée. Comme il est implicite que « les réponses sont complètes », la mutation du guide de pratique clinique au traitement standard est simple et rapide, en particulier si les étiquettes « consensus » ou « déclaration de principe » sont utilisées entre les deux en tant que mutagènes conceptuels. Au Canada, la norme de soin est supposée être un déclenchement systématique à 41 semaines. Ce commentaire est destiné à donner une pause à ceux qui ont accepté et adopté cette norme.

Abstract (English)  :

Traditionally pregnancy has been considered ‘post‐term’ at 42 completed weeks of gestation. At this gestation, if the cervix is unfavourable, debate over best practice has been between routine induction of labour and expectant management with some form of serial fetal monitoring. Popular wisdom seems to be that meta‐analysis of the available randomised controlled trials has settled the question in favour of routine induction. The largest included trial, containing over half the cases (n= 3407), was carried out in Canada and published in 1992. The results of the meta‐analysis led the Society of Obstetricians and Gynaecologists of Canada (SOGC) to issue Clinical Practice Guidelines in 1997.

The guidelines recommended that:

1. after 41 completed weeks of gestation, if the dates are certain, women should be offered elective delivery;

2. if the cervix is unfavourable, ripening should be undertaken;

and 3. if expectant management is chosen, assessment of fetal health should be initiated.

It is presumed that randomised controlled trials or, even better, meta-analyses of randomised trials, provide the best evidence to determine appropriate care. However, once information has been declared ‘the best available evidence’, particularly if that assertion is used to justify clinical practice guidelines or ‘consensus’, further inquiry may be inhibited. Since it is implied that ‘the answers are all in’, mutation from clinical practice guideline to standard of care is prompt and uncomplicated, particularly if the labels ‘consensus’ or ‘policy statement’ are used between the two as conceptual mutagens. The standard of care in Canada now is assumed to be routine induction at 41 weeks. This commentary is intended to give pause to those who have accepted and adopted this standard.
