Avec la parution des décrets sur l’amélioration des conditions de la naissance et les travaux de la conférence de consensus, nous abordons un tournant dans l’évolution de l’organisation des soins périnatals en France. La notion d’échec du plan de périnatalité de 1995 est d’actualité. La reconnaissance politique du caractère naturel et physiologique de la grossesse est concrétisée par le projet d’ouverture de « Maisons de naissance ». Ces structures sont des institutions de santé primaires gérées uniquement par des sages-femmes où peuvent accoucher des femmes présélectionnées à bas risque. Le suivi des femmes est de type accompagnement global. Les Maisons de naissance sont autonomes mais intégrées à un réseau organisé autour d’un centre hospitalier qui reçoit les femmes ou enfants lorsque l’événement de la naissance s’écarte de la physiologie. Dans ces établissements, aucun accouchement n’est déclenché, l’équipement médical est minimum, pas d’intervention médicale systématique, la femme peut choisir ses positions lors de l’accouchement. Enfin, elles sont caractérisées par la convivialité, le respect de la liberté et de l’intimité des parents.

Grenoble fait partie des villes françaises où un site pilote pourrait prendre place. Voici les prémices de l’étude de faisabilité de création d’une Maison de naissance à Grenoble. Il s’agit d’une triple étude :

- légale, qui va confronter ce projet au droit français dans lequel les Maisons de
naissance ne sont pas prévues ?
- économique, tentant de déterminer le coût de la prise en charge de la naissance à l’hôpital de Grenoble pour le comparer au coût évalué grâce à une simulation économique de Maison de naissance.
- sociologique par questionnaire auprès des praticiens en gynécologie-obstétrique de l’Isère. Il en résulte que les Centres de naissance (Maisons de naissance situées dans l’enceinte d’un hôpital) sont la seule solution pour que ce projet existe légalement, en respectant les obligations de sécurité et sans mettre la sage-femme dans des situations dangereuses.

La création de ces Centres de naissance intégrés aux réseaux apporterait une réponse aux problèmes actuels : la restructuration des maternités en réseaux pour accroître la sécurité périnatale, le manque d’obstétriciens, l’économie de la santé et l’humanisation de la naissance.

Abstract (English)  :

The publication of decrees for the improvement of birth conditions and the work sessions of the « conférence de consensus» mark a turning point in the organization of french perinatal care.

The breakdown of 1995’s perinatality project is a real topical question.
At present, there is a political awareness of the natural and physiological aspect of pregnancy. This acknowledgement is put in concrete form by a « Birthing Houses » construction project. These Houses are primary care institutions (with fewer medical aspects than in hospitals), managed by midwives and where women whose pregnancy is considered as presenting « low risk » can give birth. Their medical follow-up is a « global support » : the same midwife manages the whole pregnancy and the delivery.

Birthing Houses are independent but integrated in a network whose center is the nearest hospital center, where women or children are transfered in case of non-physiological delivery.

In these institutions : none of the deliveries is started up, medical equipment is reduced to the minimum, there is no systematic medical act and women are allowed to choose their delivery position. Birthing Houses are distinguished by their conviviality and the respect of parents’ intimity and freedom.

Grenoble is one of the French cities which are likely to be chosen as experimental sites. This report represents the early beginnings of a feasibility study for the construction of a Birthing House in Grenoble. There are three major parts in this study :

- Legal : confronting that project to the French system of law which does not anticipate structures like the Birthing Houses.
- Economic : figuring out the costs of pre to postnatal care in Grenoble Hospital, to compare it to the foreseable cost in a Birthing House (evaluated thanks to an economic simulation)
- Sociological : thanks to a survey (questionnary) among gynecology/obstetric professionals in Isère department.

As a result, Birthing Centers (i.e. Birthing Houses on the hospital premises) are the only solution for the lawful creation of this project, in the respect of safety rules and without the midwives taking to many forensic risks.

These Birthing Centers’ creation – as part of medical networks – could be answer to several current problems : the restructuration of maternity hospitals into medical networks in order to improve perinatal safety, the lack of obstetricians, health care costs and finally the necessity to humanize childbirth.
