Etude d’intervention cas-témoins sur la limitation de la prise de poids chez des femme enceintes obsèses

Ojectifs: limiter à 7 kg la prise de poids totale des femmes enceintes obèses au long de leur grossesse. Investiguer les conséquences sur l’accouchement et les résultats néonataux.
Lieu: Unité de soins anténataux [antenatal care clinics) dans le Sud de la Suède.

Population: 155 femmes dans un groupe (groupe-index), et 193 dans un groupe de contrôle.
Méthode: programme d’intervention constitué de discussions de motivation hebdomadaires et de séances de gymnastique aquatique.

Mesures : prise de poids (kg), résultats de l’accouchement et résultats néonataux.

Résultats: le groupe-index a une prise de poids significativement plus basse que le groupe de contrôle (P < 0.001). A la visite post-natale, les femmes du groupe-index avaient un poids inférieur à celui enregistré au début de leur grossesse(P < 0.001). Le pourcentage de femmes du groupe-index qui ont pris moins de 7 kg est plus élevé que celui des femmes du groupe de contrôle qui ont pris moins de 7 kg (P = 0.003). Il y avait plus de nullipares dans le groupe-index que dans le groupe de contrôle (P = 0.018). Les femmes du groupe-index ont un indice de masse corporelle significativement plus faible à la visite post-natale, comparé au groupe de contrôle (P < 0.001). Il n’y a pas de différence de poids de naissance, âge gestationnel et mode d’accouchement entre les deux groupes.

Conclusion: le programme d’intervention est efficace pour maîtriser la prise de poids pendant la grossesse et n’a pas d’effet sur les résulats néonataux.

Abstract (English)  :

Objective To minimise obese women’s total weight gain during pregnancy to less than 7 kg and to investigate the delivery and neonatal outcome.

Design A prospective case–control intervention study.

Setting Antenatal care clinics in the southeast region of Sweden.

Population One hundred fifty-five pregnant women in an index group and one hundred ninety-three women in a control group.

Methods An intervention programme with weekly motivational talks and aqua aerobic classes for obese pregnant women.

Main outcome measures Weight gain in kilograms, delivery and neonatal outcome.

Results The index group had a significantly lower weight gain during pregnancy compared with the control group (P < 0.001). The women in the index group weighed less at the postnatal check-up compared with the weight registered in early pregnancy (P < 0.001). The percentage of women in the index group who gained less than 7 kg was greater than that of women in the control group who gained less than 7 kg (P = 0.003). The percentage of nulliparous women in this group was greater than that in the control group (P = 0.018). In addition, the women in the index group had a significantly lower body mass index at the postnatal check-up, compared with the control group (P < 0.001). There were no differences between the index group and the control group regarding birthweight, gestational age and mode of delivery.

Conclusion The intervention programme was effective in controlling weight gain during pregnancy and did not affect delivery or neonatal outcome.
