Objectifs: examiner les effets des modifications de prise de poids sur le résultat de la grossesse, pour des femmes obèses.

Méthodes: étude de cohorte sur une population de 120 251 femmes enceintes obèses qui ont accouché d’un singleton vivant à terme. On a examiné les risques de quatre situations: prééclampsie, césarienne, naissances de bébés petits par rapport à leur âge gestationnel, naissance de bébés gros pour leur âge gestationnel, par rapport aux classes d’obésité et à la prise de poids totale pendant la grossesse.

Résultats: pour les femmes obèses, une prise de poids gestationnelle moindre que celle actuellement recommandée (7kg) est associée à une réduction significative des risques de prééclampsie, césarienne, et macrosome ; et à un risque accru de naissance d’un enfant petit pour son âge gestationnel. Ces résultats sont similaires quelle que soit la classe d’obésité (30-34.9, 35-35.9, and 40.0 kg/m(2)) définie par le National Institutes of Health, mais dépendent de la prise de poids de la prise de poids pendant la grossesse.

Conclusion: une prise de poids limitée ou nulle a des effets favorables sur les grossesse des femmes obèses.

Abstract (English)  :

OBJECTIVE: To examine the effect of gestational weight change on pregnancy outcomes in obese women.

METHODS: A population-based cohort study of 120,251 pregnant, obese women delivering full-term, liveborn, singleton infants was examined to assess the risk of four pregnancy outcomes (preeclampsia, cesarean delivery, small for gestational age births, and large for gestational age births) by obesity class and total gestational weight gain.

RESULTS: Gestational weight gain incidence for overweight or obese pregnant women, less than the currently recommended 15 lb, was associated with a significantly lower risk of preeclampsia, cesarean delivery, and large for gestational age birth and higher risk of small for gestational age birth. These results were similar for each National Institutes of Health obesity class (30-34.9, 35-35.9, and 40.0 kg/m(2)), but at different amounts of gestational weight gain.

CONCLUSION: Limited or no weight gain in obese pregnant women has favorable pregnancy outcomes.
