L’accouchement à domicile, une pratique alternative : connaissance, puissance, et intimité du lieu d’accouchement

Dans cet article, j’examine la démarche et les motivations en jeu lorsque les femmes, aux Etats-Unis, choisissent de contourner le modèle dominant des soins obstétriques en donnant naissance à la maison avec un groupe de soignants appelés « direct entry midwives » (sages-femmes). J’ai collecté et analysé les récits d’accouchement à domicile d’un panel de femmes (n=50) dans deux lieux différents, grâce à une « grounded theory » (méthode de recherche qualitative), l’observation des participantes et des entretiens semi-guidés et des questions ouvertes. Les résultats, interprétés selon la perspective de l’anthropologie médicale critique suggèrent que les femmes qui choisissent de donner naissance à la maison passent outre la peur du « au cas où il y a un problème», qui est à la base de l’argumentaire en faveur des naissances à l’hôpital, à l’issue de parcours individuels et sociaux complexes. Ils impliquent d’aller à l’encontre des modes de pensée officiels (« forms of authoritative knowledge »), accordant de l’importance à des connaissances acquises de manière plus centrée sur ses sensations (« embodied »), plus intuitive et plus personnelle, et au partage des connaissances s’appuyant sur la démarche de choix éclairé du patient. L’adhésion à une pensée minoritaire associée à l’expérience vécue de leur propre puissance et la préférence pour l’intimité du lieu de naissance font de l’accouchement à domicile non seulement un mouvement social minoritaire, mais aussi une forme de pratique alternative.

Abstract (English)  :

In this article, I examine the processes and motivations involved when women in the United States choose to circumvent the dominant obstetric care paradigm by delivering at home with a group of care providers called direct-entry midwives. Using grounded theory, participant observation, and open-ended, semistructured interviewing, I collected and analyzed homebirth narratives from a theoretical sample of women (n = 50) in two research locales. Findings interpreted from the perspective of critical medical anthropology suggest that women who choose to birth at home negotiate fears associated with the “just in case something bad happens“ argument that forms the foundation for hospital birth rationales through complex individual and social processes. These involve challenging established forms of authoritative knowledge, valuing alternative and more embodied or intuitive ways of knowing, and knowledge sharing through the informed consent process. Adherence to subjugated discourses combined with lived experiences of personal power and the cultivation of intimacy in the birthplace fuel homebirth not only as a minority social movement, but also as a form of systems-challenging praxis.
