Les preuves actuelles indiquent l’importance critique de plusieurs facteurs contribuant à l’amélioration des résultats périnataux : un lieu de naissance à l’environnement favorable, des soins assurés par une personne qualifiée et une continuité des soins [accompagnement global] aux femmes et nouveau-nés.

On se réfère souvent à ce niveau de soin par « soins de base » (fist-level care) qui sont plus facilement fournis par les maisons des naissance (birthing centers) et les maternités de niveau 1 (primary level health facilities).

Cependant, il y existe une quantité de preuves accumulées durant ces dernières dizaines d’années relatives à la sécurité des naissances à domicile dont les circonstances se rapprochent des soins de base (“first-level“ care). Ces études démontrent une remarquable cohérence de résultats maternels et néonataux généralement favorables, à la fois dans le temps et parmi divers groupes de populations. Ces résultats sont aussi favorables lorsque comparés à divers groupes de référence (maisons de naissance, naissances à l’hôpital et données démographiques [vital statistics]).

Ces données devraient influer les politiques en faveur des naissances prévues à domicile, incluant une politique qui prend en charge la création ou renforce une structuration des naissances à domicile en parallèle aux efforts de développement pour les naissances en structure [facility-based]. Une telle politique publique permettrait aussi d’aller dans le sens du droit fondamental des femmes à choisir la manière dont elles veulent donner naissance, en particulier quand les possibilités sont de qualité équivalente.

Abstract (English)  :

Current evidence indicates the critical importance of several factors that contribute to improved perinatal outcomes: a facilitating environment at the place of birth, skilled birth attendance, and the continuum of perinatal care for women and newborns.

This level of care is often referred to as “first-level“ care, and is most readily provided in birthing centers and primary level health facilities.

However, there is a body of evidence that has been compiled over the past several decades that addresses the safety of planned home birth, under circumstances that emulate these elements of “first-level“ care. These studies demonstrate a remarkable consistency in the generally favorable results of maternal and neonatal outcomes, both over time and among diverse population groups. These outcomes are also favorable when viewed in comparison to various reference groups (birth center births, planned hospital births, and vital statistics).

These data should influence policy in support of planned home birth, including policy that endorses building or sustaining a home birth infrastructure in parallel to the efforts to build capacity for facility-based birth. Such public policy would also be in keeping with the fundamental right of women to have choice in childbirth, particularly when options are equally good.
