Résultat des accouchements prévus à domicile par rapport aux accouchements à l’hôpital en Suède entre 1992 et 2004. Une étude basée sur le registre de population

Objectif : L’objectif de cette étude basée sur la population était de mesurer le risque d’issue défavorable pour la mère et l’enfant lors des accouchements prévus à domicile, dans une population suédoise, indépendamment du lieu effectif où s’est produite la naissance, à la maison ou à l’hôpital après transfert.

Méthode : Etude basée sur la population utilisant des données provenant du Registre médical suédois des naissances (the Swedish Medical Birth Register).
Suède 1992-2004. Un total de 897 accouchements prévus à domicile ont été comparés à un groupe choisi au hasard de 11.341 naissances prévues en milieu hospitalier.
Comparaison de la prévalence de la mortalité et de la morbidité chez les mères et les enfants, situations d’urgence, accouchement instrumental et par opération et déchirures du périnée.

Résultats : Pendant cette période, en Suède, le taux de mortalité néonatale a été de 2,2 pour mille dans le groupe d’accouchement à domicile et de 0,7 dans le groupe hospitalier (RR 3,6, IC 95% 0.2 14.7). Aucun cas de complications d’urgence n’a été trouvé dans le groupe des accouchements à domicile. Le risque de rupture des sphincters a été plus faible dans le groupe des accouchements prévus à domicile (RR 0,2, IC 95% 0.0-0.7). Le risque de césarienne (RR 0,4, IC 95%: 0,2 à 0.7) ou d’extraction instrumentale (RR 0,3, IC 95%: de 0,2 à 0,5) était significativement inférieur dans le groupe d’accouchement prévu à domicile.

Conclusion : En Suède, entre 1992 et 2004, la mortalité intrapartum et néonatale pour les accouchements prévus à domicile était de 2,2 pour mille. La proportion est plus élevée par rapport aux naissances à l’hôpital, mais aucune différence statistiquement significative n’a été trouvée. Les femmes du groupe des accouchements à domicile ont eu plus souvent une naissance spontanée sans intervention médicale et étaient moins susceptibles de subir des traumatismes du plancher pelvien.

Abstract (English)  :

OBJECTIVE: The aim of this population-based study was to measure the risk of adverse outcome for mother and child in planned home births in a Swedish population irrespective of where the birth actually occurred, at home or in hospital after transfer. DESIGN: A population-based study using data from the Swedish Medical Birth Register. SETTING: Sweden 1992-2004. PARTICIPANTS: A total of 897 planned home births were compared with a randomly selected group of 11,341 planned hospital births. MAIN OUTCOME MEASURES: Prevalence of mortality and morbidity among mothers and children, emergency conditions, instrumental and operative delivery and perineal lacerations were compared. RESULTS: During this period in Sweden the neonatal mortality rate was 2.2 per thousand in the home birth group and 0.7 in the hospital group (RR 3.6, 95% CI 0.2-14.7). No cases of emergency complications were found in the home birth group. The risk of having a sphincter rupture was lower in the planned home birth group (RR 0.2, 95% CI 0.0-0.7). The risk of having a cesarean section (RR 0.4, 95% CI 0.2-0.7) or instrumental delivery (RR 0.3, 95% CI 0.2-0.5) was significantly lower in the planned home birth group. CONCLUSION: In Sweden, between 1992 and 2004, the intrapartum and neonatal mortality in planned home births was 2.2 per thousand. The proportion is higher compared to hospital births but no statistically significant difference was found. Women in the home birth group more often experienced a spontaneous birth without medical intervention and were less likely to sustain pelvic floor injuries.
