Diagnostic non invasif d’aneuploïdie fœtale par le séquençage de l’ADN « shotgun » à partir du sang maternel

Nous avons séquencé directement l’ADN non cellulaire libre obtenu du plasma des femmes enceintes par une technologie de séquençage à haut débit « shotgun » et nous avons obtenu en moyenne 5 millions de séquences par échantillon. Ceci nous a permis d’évaluer la présence surnuméraire ou le déficit de chromosomes d’un fœtus aneuploïde. L’approche de séquençage est indépendante du polymorphisme génétique et par conséquent elle est applicable universellement pour la détection non-invasive d’aneuploïdies fœtales. En utilisant cette méthode, nous avons identifié avec succès neuf cas de Trisomie 21 (syndrome de Down), deux cas de Trisomie 18 (syndrome d’Edward) et un cas de Trisomie 13 (syndrome de Patau) dans une cohorte de 18 grossesses aneuploïdes et normales ; la trisomie a été détectée à partir d’âge gestationnel de 14 semaines d’aménorrhée. Le séquençage direct nous a également permis d’étudier les caractéristiques de l’ADN plasmatique libre (non cellulaire) et nous avons prouvé que cet ADN est enrichi en séquences dérivées de nucléosomes.

Abstract (English)  :

We directly sequenced cell-free DNA with high-throughput shotgun sequencing technology from plasma of pregnant women, obtaining, on average, 5 million sequence tags per patient sample. This enabled us to measure the over-and underrepresentation of chromosomes from an aneuploid fetus. The sequencing approach is polymorphismindependent and therefore universally applicable for the noninvasive detection of fetal aneuploidy. Using this method, we successfully identified all nine cases of trisomy 21 (Down syndrome), two cases of trisomy 18 (Edward syndrome), and one case of trisomy 13 (Patau syndrome) in a cohort of 18 normal and aneuploid pregnancies; trisomy was detected at gestational ages as early as the 14th week. Direct sequencing also allowed us to study the characteristics of cell-free plasma DNA, and we found evidence that this DNA is enriched for sequences from nucleosomes.

• Le diagnostic se fait à la 14e semaine alors que la méthode ISET permet de diagnostiquer à 8 semaines.
• Le prélèvement est fait après la BVC ou l’amniocentèse (probablement pour créer une condition de quantité accrue d’ADN fœtal libre dans le plasma maternel).On ne sait malheureusement pas si le résultat aurait été le même dans le cas de prélèvement avant la méthode invasive.
• Ils analysent par séquençage 5 millions de fragments d’environ 200bp. Combien de temps cela prend-il ?
• Des 10 millions de fragments obtenus, seulement la moitié est utilisable (donc avec une séquence qui matche celle du génome), ce qui montre le nombre énorme de séquences non spécifiques. Et, en effet, quand on regarde la Fig 1A, la méthode donne des aneuploidies "fausses" pour les chromosomes 10, 15, 20, 16, 17, 22, et 19. Quelle chance que les chromosomes 13, 18 et 21 soient parmi ceux sans résultats non spécifiques ! Mais est-ce que cela est reproductible ?
• L’analyse est a posteriori, comme écrit dans les Méthodes (ils savaient au moment du test quels échantillons étaient pathologiques et connaissaient le diagnostic de trisomie)
