OBJECTIFS : Comparer les expériences de travail et accouchements de femmes ayant donné naissance à domicile sans complication avec les expériences de femmes ayant donné naissance sans complication dans une maison de naissance (birth center).
CONCEPTION : étude descriptive utilisant des questionnaires postaux de 1 à 6 mois après la naissance d’un échantillon consécutif de femmes lors du post partum.
CADRE : On a recruté des femmes dans une maison de naissance et dans trois cabinets de sages-femmes situés dans une zone urbaine des Pays-Bas entre septembre et décembre 2003.
PARTICIPANTS : 193 femmes, 129 ont donné naissance à domicile et 64 dans une maison de naissance.
RESULTATS : [Les femmes du] groupe à domicile ont perçu moins de douleur (moyenne du score naissance à domicile 6,291, naissance en maison de naissance 6,977), ont deamndé moins de soulagement médicamenteux de la douleur (naissance à domicile 7.9%, naissance en maison de naissance 21.9%), ont trouvé qu’elles connaissaient mieux leur sage-femme (naissance à domicile 36%, naissance en maison de naissance 10% « la connaissaient bien »), et ont mieux noté l’environnement de la naissance (moyenne du score naissance à domicile 4.70, naissance en maison de naissance 4.01). De plus, le groupe naissance en maison de naissance a valorisé la sécurité, le fait d’avoir une aide médicale disponible et la commodité, alors que le groupe naissance à domicile a accordé plus d’importance au fait que la maison leur était plus sûre et vecteur de confiance, d’avoir leur propre endroit et effets personnels, et de se sentir à leur aise et détendues.
CONCLUSIONS PRINCIPALES : Le fait de comprendre l’expérience du travail et de l’accouchement d’une femme permet aux soignants de continuer à améliorer la qualité des soins liés à la maternité. L’environnement peut avoir un effet positif sur l’expérience d’une femme de l’accouchement. Des recommandations pouvant être appliquées à toutes les femmes enceintes et en train d’accoucher ont été proposées.
IMPLICATIONS SUR L’EXERCICE : l’identification et la compréhension des facteurs environnementaux qui rendent l’expérience du travail et de la naissance plus positive devraient être incorporés à l’information et la préparation à la naissance.

Abstract (English)  :

OBJECTIVE: to compare the labour and birth experiences of women who delivered at home without complications with the experiences of women who delivered in a birth centre without complications. DESIGN: a descriptive study using postal questionnaires at 1-6 months after birth of a consecutive sample of postpartum women.
SETTING: women were recruited from one birth centre and three midwifery practices in an urban area of the Netherlands between September and December 2003.
PARTICIPANTS: 193 women; 129 delivered at home and 64 delivered in the birth centre.
FINDINGS: the home-birth group perceived less pain (mean score home birth 6.291, birth-centre birth 6.977), desired less pain-relieving medication (home birth 7.9%, birth-centre birth 21.9%), believed they knew their midwife better (home birth 36%, birth-centre birth 10% ’knew her well’), and rated their birth setting ’higher’ than the birth-centre group (mean score home birth 4.70, birth-centre birth 4.01). Furthermore, the birth-centre group emphasised safety, having medical help available, and convenience, whereas the home-birth group placed more importance on the home being trustworthy and dependable, having their own place and belongings, and feeling comfortable and relaxed.
KEY CONCLUSIONS: having an understanding of a woman’s labour and delivery experience allows health-care providers to continue to improve the quality of maternity care. The environment can have a positive effect on a woman’s birth experience; recommendations have been proposed that can be applied to all pregnant and labouring women.
IMPLICATIONS FOR PRACTICE: identification and understanding of the factors in the environment that make the labour and birth experience more positive should be incorporated into the education and preparation for an upcoming birth.
