Objectif
Déterminer les facteurs de risque de lésions du sphincter anal lors d’un accouchement vaginal avec extraction

Cadre et conception
Une étude observationnelle basée sur la population.

Population
Les 21 254 femmes accouchées avec extraction par le vide et 7 478 accouchées avec une pince, tirées de la base de données obstétricale nationale néerlandaise précédemment validée pour les années 1994 à 1995.

Méthodes
Par lésion du sphincter anal, on entend toute blessure, partielle ou complète, du sphincter anal. Les facteurs de risque ont été déterminés par analyse de régression logistique multivariée.

Principaux critères de jugement
Facteurs obstétricaux individuels, p. ex. poids de la naissance fœtale, durée du deuxième stade, etc.

Résultats
Une lésion du sphincter anal est survenue dans 3,0% des extractions avec ventouse et dans 4,7% des accouchements au forceps. La primiparité, la position occipito-gastrique et le poids du fœtus étaient associés à un risque accru de lésion du sphincter anal dans les deux types d’accouchement vaginal, tandis que la durée du deuxième stade était associée à un risque accru uniquement lors des extractions sous vide. L’épisiotomie médiolatérale est significativement protégée contre les lésions du sphincter anal lors de l’extraction sous vide (OR 0,11, IC 95%: 0,09–0,13) et lors de l’accouchement au forceps (OR 0,08, IC 95%: 0,07–0,11). Le nombre d’épisiotomies médiolatérales nécessaires pour prévenir une lésion d’un sphincter lors d’extractions sous vide était de 12, alors que 5 épisiotomies médiolatérales pourraient prévenir une lésion d’un sphincter lors de l’accouchement au forceps.

Conclusions
La primiparité et la présentation occipito-venteuse sont des facteurs de risque importants pour la survenue d’une lésion du sphincter anal lors d’un accouchement vaginal avec extraction. L’effet protecteur hautement significatif des épisiotomies médiolatérales dans les deux types d’accouchement par voie vaginale opérante justifie les conclusions selon lesquelles ce type d’épisiotomie devrait être utilisé systématiquement lors de ces interventions pour protéger les sphincters anaux.

Abstract (English)  :

Objective
To determine the risk factors for anal sphincter injuries during operative vaginal delivery.

Setting and design
A population-based observational study.

Population
All 21 254 women delivered with vacuum extraction and 7478 women delivered with forceps, derived from the previously validated Dutch National Obstetric Database from the years 1994 to 1995.

Methods
Anal sphincter injury was defined as any injury, partial or complete, of the anal sphincters. Risk factors were determined with multivariate logistic regression analysis.

Main outcome measures
Individual obstetric factors, e.g. fetal birthweights, duration of second stage, etc.

Results
Anal sphincter injury occurred in 3.0% of vacuum extractions and in 4.7% of forceps deliveries. Primiparity, occipitoposterior position and fetal birthweight were associated with an increased risk for anal sphincter injury in both types of operative vaginal delivery, whereas duration of second stage was associated with an increased risk only in vacuum extractions. Mediolateral episiotomy protected significantly for anal sphincter damage in both vacuum extraction (OR 0.11, 95% CI 0.09–0.13) and forceps delivery (OR 0.08, 95% CI 0.07–0.11). The number of mediolateral episiotomies needed to prevent one sphincter injury in vacuum extractions was 12, whereas 5 mediolateral episiotomies could prevent one sphincter injury in forceps deliveries.

Conclusions
Primiparity and occipitoposterior presentation are strong risk factors for the occurrence of anal sphincter injury during operative vaginal delivery. The highly significant protective effect of mediolateral episiotomies in both types of operative vaginal delivery warrants the conclusions that this type of episiotomy should be used routinely during these interventions to protect the anal sphincters.
