La listériose materno-néonatale (MN) regroupe les listérioses diagnostiquées
pendant la grossesse ou le premier mois de vie. Leur nombre a été
divisé par 10 de 1986 à 2006 (500 cas/an vs 50 cas/an). Quatre cent et un
cas de listériose MN notifiés par la déclaration obligatoire entre 1999 et
2006 ont été analysés. Selon le moment du diagnostic et l’issue de la
grossesse, ils se répartissent en listériose néonatale (59 %), mort in utero
(26,5 %) et infection maternelle isolée (14,5 %). La mortalité in utero (26 %)
diminue selon l’âge gestationnel : 75 % avant 22 semaines d’aménorrhée
(SA), 52 % à 22-27 SA, 12 % à 28-31 SA, 5 % à 32-36 SA, 1 % 37-41 SA. La
mortalité néonatale (9 %) diminue également selon l’âge gestationnel
(41 % à 22-27 SA, 19 % à 28-31 SA, 5 % à 32-36 SA, 1 % 37-41 SA) et selon
le prélèvement à l’origine du diagnostic : nouveau-nés avec infection invasive
(16 %), nouveau-nés avec mère infectée (8 %), liquide d’aspiration
gastrique (4 %), prélèvements superficiels (0 %). Soixante-huit pour cent
des femmes ont consommé pendant leur grossesse des aliments dont on
recommande d’éviter la consommation. Le maintien des mesures de contrôles
de la filière agro-alimentaire, qui ont montré leur efficacité dans la
réduction des cas listériose MN, est essentiel. Il serait utile

Abstract (English)  :

Pregnancy associated and neonatal listeriosis in France:
trend from 1984 to 2006
Materno-neonatal (MN) listeriosis includes listeriosis diagnosed during
pregnancy or the first month of life. From 1986 to 2006, the annual number
of cases of MN listeriosis was decreased tenfold (550 cases/year vs 50 cases/
year). Four hundred and one cases of MN listeriosis mandatorily reported to
French Institute of Public Health Surveillance from 1999 to 2006 were
analyzed. Depending on diagnosis and end of pregnancy, they are subdivided in
three groups: neonatal listeriosis (59%), foetal death (26.5%), and maternal
infection (14.5%). Foetal loss (26%) decreases according to gestational age:
75% before 22 weeks of pregnancy, 52% before 22-27 weeks, 12% between
28-31 weeks, 5% between 32-36 weeks, 1% between 37-41 weeks. Neonatal
mortality (9%) also decreases according to gestational age (41% before
22-27 weeks, 19% between 28-31 weeks, 5% between 32-36 weeks, 1% between
37-41 weeks) and the nature of samples giving the diagnosis of listeriosis:
neonate with invasive infection (16%), maternal infection (8%), gastric liquid
(4%), superficial samples (0%). Sixty-eight per cent of the cases had consumed
products which were not recommended during pregnancy. Control
measures in food-production industry that have demonstrated their efficiency
in MN listeriosis case reduction have to be maintained. It would be useful to
evaluate the reasons for non compliance with recommendations to avoid
at-risk foods.
