Résumé (français)  :

Objectif. Analyser les données de la littérature sur la comparaison de deux politiques de réalisation de l’épisiotomie : utilisation libérale versus restrictive. Rechercher et étudier des indications spécifiques.

Méthodes. La base Medline a été consultée pour les références parues entre 1970 et 2005. Les publications ont été sélectionnées en croisant les mots-clés episiotomy et selective ou restrictive et routine ou liberal. Puis, chaque indication potentielle a été croisée avec episiotomy.

Résultats. Une politique libérale de la pratique de l’épisiotomie n’est pas supérieure à une politique restrictive. Bien au contraire, une littérature à fort niveau de preuve plaide en faveur de l’abandon de l’épisiotomie dite de routine dans les accouchements à bas risque. Concernant l’étude des indications spécifiques, l’analyse de la littérature ne permet de retenir qu’une seule indication de l’épisiotomie systématique. Il s’agit d’une longueur périnéale minime ou brièveté périnéale lorsque la distance fourchette-centre de l’anus est inférieure à 3 cm. Toutefois, le sens clinique de l’obstétricien pourra l’amener chaque fois que nécessaire à agrandir l’orifice périnéo-vaginal afin de tenter d’améliorer les conditions d’un accouchement à risque.

Conclusion. L’analyse de la littérature ne montre pas d’avantages à une politique libérale d’épisiotomie par rapport à une utilisation restrictive tant sur le versant fœtal (grade C) que maternel (grade A). De nombreuses situations obstétricales considérées à risques n’indiquent pas systématiquement l’épisiotomie. Mais il existe des circonstances au cours desquelles un jugement clinique prudent et pertinent dictera à l’obstétricien la réalisation d’une épisiotomie.

Abstract (English)  :

Routine versus selective episiotomy.
Purpose. To analyze the literature comparing two different policies for episotomy practice: liberal versus restrictive use. To search and discuss specific indications for episiotomy.

Methods. The Medline base was analyzed from 1970 to 2005. The articles where selected by using the key word episiotomy and selective or restrictive and routine or liberal. Every potential indications was crossed with episotomy.

Results. A policy implying a liberal practice of episiotomy is not better compared to a restrictive policy. The evidence-based medical literature favors avoiding routine episiotomy in low risk deliveries.

Data are quite scarce concerning the different specific indications for episiotomy, and finally we can retain only one specific indication which is the short perineum when the distance between the fourchette and the center of the anus is less than 3 cm. Nevertheless, in order to improve delivery conditions obstetricians can advisably use episiotomy in accordance with their clinical assessment.

Conclusion. There is no evidence in the literature favoring a liberal policy over a restrictive policy for the use of episiotomy, both in terms of fetal (Grade C) and maternal (Grade A) indications. A number of obstetrical situations considered as at risk do not systematically indicate an episiotomy. There are however circumstances in which a pertinent and prudent clinical assessment will lead the obstetrician to use an episiotomy.
