Résumé (français)  :

Objectif. Essayer de définir concernant l’épisiotomie, les soins les plus appropriés, les traitements les plus adaptés de la douleur et le devenir des fils.

Matériel et méthodes. Recherche à travers la base de données Medline et de la Cochrane Library d’articles sur le sujet entre 1990 et 2005.

Résultats. Concernant les soins locaux, il est primordial d’avoir la meilleure hygiène possible pour favoriser la cicatrisation ; aucun soin spécifique n’a été retenu. Le traitement de la douleur par les moyens non médicaux ainsi que par les moyens locaux n’est pas très efficace. Concernant les moyens médicaux, les anti-inflammatoires non stéroïdiens ont été les plus étudiés et certains sont efficaces sur la douleur que ce soit per os ou par voie rectale. Certains sont même utilisables pendant l’allaitement maternel. Le paracétamol n’a pas fait l’objet de suffisamment d’études pour que l’on puisse parler de son efficacité.

Conclusion. Le personnel soignant doit insister au cours du séjour en maternité pour que chaque patiente se prenne en charge et ait une hygiène satisfaisante. La douleur de l’épisiotomie est primordiale à traiter et les moyens médicaux sont les plus efficaces. Le traitement doit être adapté à chaque femme en fonction de sa tolérance et de son mode d’allaitement.

Abstract (English)  :

Post-delivery local care after episiotomy.
Objectives. Attempt to define the most appropriate care after an episiotomy, the best-suited treatments of the pain of episiotomy, and examine the course of repair stitches.

Material and methods. A survey of the literature found on Medline and on the Cochrane Library between 1990 and 2005, and dealing with these questions was carried out.

Results. As regards local care, the best possible personal hygiene is a key to healing, but no specific treatment has been accepted. The treatment of pain by non-medical means as well as topically is not very efficient. Nonsteroidal antiinflammatory drugs were the most studied among medical treatments and have shown an effect on pain (whether per os or by rectal route), some of them can be used during maternal breastfeeding. Acetaminophen has not been sufficiently studied to assess its effectiveness.

Conclusion. The medical staff must emphasize the importance of self care and personal hygiene. The pain of episiotomy is a crucial element to treat, and it is most effectively done medically. The treatment must be adapted to each patient depending on her tolerance and whether she is breastfeeding or not.
