Objectif
Comparaison de la morbidité maternelle immédiate après extraction fœtale par spatules de Thierry et par ventouses obstétricales.

Patientes et méthodes
Étude rétrospective, menée à l’hôpital la Conception, Marseille, France. La totalité des naissances par extraction instrumentale entre novembre 2003 et mai 2005, soit 264, a été identifiée. Des analyses uni- et multivariées ont été conduites de façon comparative. Ainsi, ont été évalués, le taux de lésion périnéale, le taux d’hémoglobine en post-partum, ainsi que la durée d’hospitalisation après l’accouchement pour les extractions par spatules de Thierry et par ventouses obstétricales.

Résultats
Parmi les 264 extractions instrumentales étudiées, 96 (36,4 %) l’ont été par ventouse et 168 (63,6 %) l’ont été par spatules de Thierry. L’extraction par spatules de Thierry a été pratiquée plus fréquemment chez des primipares que chez les multipares (<0,001). Les extractions par spatules de Thierry étaient plus fréquemment réalisées sous anesthésie péridurale (p =0,05), avaient une première et une seconde partie du travail plus longue (p =0,002 et p =0,03, respectivement). L’analyse statistique ne suggère aucune différence dans l’incidence des lésions du sphincter anal entre les 168 extractions par spatules et les 96 extractions par ventouses (4,2 et 3,2 % respectivement, p =0,67). En revanche, le taux d’hémoglobine en post-partum était statistiquement plus bas dans le groupe d’extraction par spatules de Thierry que dans celui par ventouse (p < 0,001). La durée moyenne d’hospitalisation était supérieure dans le groupe des extractions par spatules de Thierry (5,6jours) que dans celui par ventouse (4,7jours) (p < 0,001).

Discussion et conclusion
L’incidence des lésions périnéales du troisième degré était similaire pour les extractions par spatules de Thierry et par ventouse. Mais la naissance par ventouse est associée dans notre travail à une perte sanguine maternelle plus faible et une durée d’hospitalisation plus courte.

Abstract (English)  :

Objective
To compare maternal morbidity after Thierryʼs spatulas or vacuum-assisted deliveries.

Patients and methods
Retrospective study, at the French hospital la Conception, in Marseilles. All successful instrumental deliveries between November 2003 and May 2005 were reviewed, that is to say 264. Univariate and multivariate analysis were performed comparing maternal morbidity in the two groups. The primary outcome measure was perineal trauma. Secondary outcomes were blood loss and duration of hospitalization.

Results
Among the 264 deliveries, there were 96 vacuum deliveries and 168 Thierryʼs spatulas extraction. Thierryʼs spatulas were use more often in nulliparous patient (<0.001). Patients in the group of Thierryʼs spatulas have a higher rate of epidural analgesia (p =0.05), a longer duration of first (p =0.002) and second stage of labor (p =0.03). There was no difference in incidence of sphincter tears between women who underwent Thierryʼs spatulas and those who underwent vacuum delivery with respective incidence of 4.2 and 3.2% (p =0.67). There was a significant difference in post-partum hemoglobin value with a higher blood loss in the group of Thierryʼs spatulas (<0.001). Mean duration of hospitalization was longer in the group with Thierryʼs spatulas (5.6days) than in the group who underwent vacuum delivery (4.7days) (p < 0.001).

Discussion and conclusion
Incidence of third degree tears was similar between the vacuum and Thierryʼs spatulas group. Deliveries with vacuum are associated with less blood loss and a shorter hospitalization stay.
