Objectif
Comparer la morbidité materno-fœtale de l’extraction par spatules de Teissier longues à celle de la ventouse obstétricale de type Minicup®.

Matériel et méthodes
Étude rétrospective monocentrique du 1er janvier 2003 au 31 juillet 2008. Durant cette période, toutes les extractions instrumentales réalisées par spatules de Teissier sur singletons en présentation céphalique à terme (≥ 37 semaines d’aménorrhées) ont été recensées. Le groupe témoin a été constitué de chaque extraction par ventouse précédant chaque cas.

Résultats
Nous avons recensé 69 cas d’extraction par spatules de Teissier longues. Les caractéristiques maternelles ne retrouvaient pas de différence significative entre les deux groupes. Deux extractions par spatules se sont compliquées de déchirures périnéales du troisième degré, mais sans différence significative avec le groupe ventouse. Un taux d’épisiotomie de 15,9 % accompagnait les extractions par spatules, contre 11,6 % dans le groupe ventouse (non significatif). La seule donnée significativement différente était la durée d’extraction supérieure dans le groupe ventouse (4,95minutes contre 3,4minutes dans le groupe spatules, p =0,007). La morbidité fœtale était comparable dans les deux groupes.

Conclusion
Au terme de cette étude, aucune différence significative de morbidité materno-fœtale n’a été retrouvée entre le groupe d’extraction par spatules de Teissier et le groupe ventouse. De plus et contrairement à ce que décrit la littérature sur les spatules de Thierry, les spatules de Teissier longues peuvent être utilisées dans le cadre d’une politique restrictive du recours à l’épisiotomie, ce qui va dans le sens des recommandations professionnelles et de la demande des parturientes. Une étude de plus ample envergure devra être réalisée pour confirmer ces données préliminaires.

Abstract (English)  :

Objective
To compare fetal and maternal morbidities between operative deliveries by long Teissier’s spatulas and Minicup® vacuum extractor.

Material and methods
A retrospective study was conducted from January 2003 to July 2008 at the maternity ward, Besançon teaching hospital. Operative deliveries in term cephalic singleton pregnancies performed by Teissier’s spatulas (case group) were compared to previous deliveries by vacuum extractor Minicup® (control group).

Results
During the study period, 69 operative deliveries by Teissier’s spatulas have been performed. No significant difference was found between the two groups in terms of maternal characteristics. Two third-degree perineal tears occurred following delivery by Teissier’s spatulas with no third-degree tear in the vacuum extractor group (p =ns).

The episiotomy rate in the Teissier’s spatulas group was 15,9% compared to 11.6% in the vacuum extractor group (p =ns). Duration of operative delivery was significantly shorter in the Teissier’s spatulas group (3.4min vs 4.95min; p =0.007). Fetal morbidity was identical in the two groups.

Conclusion
This study found no significant difference in terms of fetal and maternal morbidities between operative deliveries by Teissier’s spatulas and vacuum extractor.

Moreover, as opposed to Thierry’s spatulas, the long Tessier spatulas can be adequately used in accordance with pateint’s wish and pratice guidelines recommending a policy of restrictive episiotomy. However, a larger study is needed to confirm these preliminary results.
