Les infections nosocomiales représentent un enjeu en terme de santé publique. Elles sont responsables d’une augmentation de la morbidité et occasionnent un coût important pour la collectivité. Une surveillance épidémiologique est instaurée au sein des services de gynécologie obstétrique et maternité, elle a pour but d’établir le taux d’infections nosocomiales et les facteurs de risque associés à leur survenue. Il s’agit d’une enquête d’incidence sur la période 2000–2005. Les fiches de surveillance sont remplies pour chaque accouchement. Les suspicions d’infections nosocomiales sont analysées en revues de morbidité effectuées tous les trimestres. Une requête auprès du département d’information médicale de l’hôpital a permis de rechercher des variables non présentes sur le questionnaire initial et d’effectuer une analyse plus complète. Le nombre d’IN s’élève à 118 sur 9526 accouchements, correspondant à un taux d’incidence de 1,24 %. Après un accouchement par voie basse, les facteurs de risque retrouvés sont les suivants : l’épisiotomie ou la déchirure périnéale, l’anesthésie péridurale, l’infection urinaire à l’entrée et les manœuvres instrumentales. Après une césarienne, les facteurs de risque sont : l’anesthésie générale et l’absence d’antibioprophylaxie. Le taux d’IN en maternité observé au CHU de Brest sur la période et la tendance à la diminution des infections nosocomiales observées au cours de la période laisseraient apparaître l’intérêt de la surveillance quant à la survenue des infections nosocomiales en maternité.

Abstract (English)  :

Hospital infections are at stake in terms of public health. They are responsible for increase in morbidity and involve the community in high costs. Epidemiologic surveillance has been initiated in the departments of gynecology, obstetrics and maternity with a view to making out the rate of hospital-acquired infections and the risk factors associated to them. It is an incidence survey over a period from 2000 to 2005. Surveillance slips are filled in for every childbirth. All suspicions of hospital infections are analysed in morbidity reviews every trimester. A request to the Medical Information Department of the hospital has allowed to look for variables which were not mentioned on the initial questionnaire and so carry out a more complete analysis. The number of hospital infections amounts to 118 over 9526 childbirths, corresponding to an incidence rate of 1.24%. After vaginal delivery the encountered risk factors are: episiotomy or perineal trauma, epidural anesthesia, urinary infection and the use of tools. After a caesarean section the risk factors are: general anesthesia and lack of antibioprophylaxy. The rate of hospital infections in the maternity department at Brestʼs centre hospitalier universitaire (CHU) during the considered period and the observed tendency to a decreasing of hospital infections over the same period apparently denotes the interest of surveillance in matter of hospital infections in maternity.
