Objectif : Étudier les causes du défaut de diagnostic des hémorragies du post-partum.

Méthode : Étude rétrospective de 5 517 dossiers de patientes ayant accouchées par voie basse à la Clinique Gynécologique et Obstétricale du CHU de Rouen entre le 1/1/1997 et le 30/09/99. Quatre-ving-dix patientes (1,63 %) ayant accouché sans hémorragie diagnostiquée ont présenté une anémie du post-partum, avec une différence d’hématocrite d’au moins 10 points entre le bilan réalisé avant l’accouchement et le post-partum, ce qui correspond à une hémorragie supérieure à 1 litre. Notre avons étudié et comparé ce groupe aux hémorragies diagnostiquées et au reste de la population. L’analyse statistique a été réalisée à l’aide du test du Khi 2 ou de Fischer en fonction des effectifs et des comparaisons de moyennes par le calcul de z.

Résultats : Les facteurs de risque augmentés significativement correspondent à la primiparité, la présence d’une anémie avant l’accouchement, la réalisation d’un déclenchement, la présence d’une anesthésie loco-régionale, l’utilisation d’ocytocine, un travail long avec une phase active longue, une hyperthermie pendant le travail, la réalisation d’une épisiotomie et un délai prolongé entre la naissance de l’enfant et le début de la suture.

Conclusions : L’absence de diagnostic semble imputable au fait que l’estimation visuelle simple des saignements est un indicateur très médiocre notamment quand aucun moyen de recueil et de quantification n’est utilisé. De plus, il faut redéfinir les facteurs influençant la survenue d’une hémorragie en insistant sur la primiparité et ce qu’elle implique.

Abstract (English)  :

Missed diagnosis of hemorrhage in the postpartum period.
Objective: The purpose of this work was to search for the reasons why the diagnosis of post-partum hemorrhage may be missed.

Patients and methods: We reviewed retrospectively the files 5,517 vaginal delivery patients cared for in our Gynecology and Obstetrics Unit at the Rouen University Hospital between January 1, 1997 and September 30, 1999. Among these patients, 90 (1,63%) developed anemia during the post-partum period with a 10-point fall in the hematocrit from the pre-delivery level, which corresponds to a 1 liter loss of blood, but for whom no diagnosis of hemorrahage was made. We compared this group with the population of patients who had had a diagnosis of hemorrhage. The chi-squared or Fischer’s exact text were used where appropriate to compare means and calculate z.

Results: Significant risk factors were: primiparity, anemia before delivery, labor induction, locorregional anesthesia, use of ocytocin, long labor with a long active phase, fever during labor, episiotomy and prolonged delay between delivery and onset of suture.

Conclusions: Missed diagnosis appeared to be related to use of visual assessment to determine the degree of bleeding, a very mediocre indicator, particularly when no means of blood collection or quantification is used. In addition, factors having an impact on the development of hemorrhage should be revisited with particular attention given to primiparous patients.
