Le réseau «Sécurité naissance-Naître ensemble » des Pays de la Loire a organisé, avec les pédiatres et les sages-femmes des 24 maternités de la région, des séances d’élaboration de recommandations sur la prise en charge du nouveau-né normal dans les six premières heures, en 2005 et 2006. Ce thème a été choisi en raison des nouvelles attitudes de « bien-traitance » des enfants, une fois leur sécurité assurée. Des points faisant consensus ont été retenus: définition de la normalité à la naissance, limitation de l’introduction de sondes buccopharyngées et nasales, environnement du nouveau-né, gestes à différer, examen clinique à la naissance… D’autres points ont fait débat et ont été laissés à l’appréciation des équipes: moment du clampage du cordon, dépistage de l’atrésie de l’œsophage par l’introduction d’une sonde œsogastrique, indications et modalités et du peau-à-peau et soins d’yeux. La diffusion a été assurée par envoi télématique et lors des Journées scientifiques régulières du réseau, auprès des pédiatres, sages-femmes et gynéco-obstétriciens. Ces recommandations ont été évaluées par un audit déclaratif montrant une bonne acceptabilité et une bonne faisabilité de ceux qui ont répondu à l’audit (15/24). Un audit d’observation des pratiques en salle de naissance devra être organisé.

Abstract (English)  :

Perinatal “Sécurité naissance-Pays de la Loire” (France) network organized together with paediatricians and midwives of the 24 maternity units in the region of meetings to propose, in 2005 and 2006, recommendations on the care of healthy newborns in the six first hours. This theme was chosen because of new attitudes have for the good treatment of children, once their safety assured. Consensus points which have been identified: definition of normality at birth, limiting the introduction of oral and nasal pharynx suction, the environment of the newborn, delayed actions, clinical examination at birth… Other discussion points were made and were left to the discretion of teams: the time of cord clamping, testing of atresia of the esophagus by the introduction of a gastric suctioning, indications and modalities and skin to skin and eye care. Sending telematics days and at regular scientific network with pediatricians, midwives and gynecologists-obstetricians ensured dissemination. These recommendations have been assessed by an audit statement shows good acceptability and good feasibility of those who responded to the audit (15/24). An audit observation practices birth room should be provided.
