Objet : Nous avons cherché à compiler la littérature scientifique sur la sécurité maternelle et infantile des accouchements prévus à domicile comparés aux accouchements prévus en structure hospitalière.

Conception de l’étude: Nous avons inclus les publications “peer-reviewed” en langue anglaise dans les pays occidentaux à propos des conséquences maternelles et infantiles en fonction des lieux prévus de naissance. Nous avons calculé le rapport des cotes (odds ratio) avec un intervalle de confiance de 95%.

Résultats : Les accouchements prévus à domicile sont associés à moins d’interventions médicales tels analgésie péridurale, monitoring fœtal, extraction instrumentale, épisiotomie. Les femmes ont été moins concernées par des déchirures, hémorragies, et infections. Les résultats néonatals ont montré moins de bébés prématurés, de faible poids et d’assistance respiratoire. Bien qu’on retrouve les mêmes taux de mortalité périnatale pour les accouchements prévus à domicile et ceux prévus à l’hôpital, les accouchement prévus à domicile sont associés à des taux de mortalité néonatale significativement plus élevés

Conclusion : Un moindre interventionnisme médical lors des naissances à domicile est associé à un taux de mortalité néonatale multiplié par trois.

Abstract (English)  :

Objective

We sought to systematically review the medical literature on the maternal and newborn safety of planned home vs planned hospital birth.

Study design

We included English-language peer-reviewed publications from developed Western nations reporting maternal and newborn outcomes by planned delivery location. Outcomes’ summary odds ratios with 95% confidence intervals were calculated.

Results

Planned home births were associated with fewer maternal interventions including epidural analgesia, electronic fetal heart rate monitoring, episiotomy, and operative delivery. These women were less likely to experience lacerations, hemorrhage, and infections. Neonatal outcomes of planned home births revealed less frequent prematurity, low birthweight, and assisted newborn ventilation. Although planned home and hospital births exhibited similar perinatal mortality rates, planned home births were associated with significantly elevated neonatal mortality rates.

Conclusion

Less medical intervention during planned home birth is associated with a tripling of the neonatal mortality rate.
