Etude prospective sur 5 ans sur les risques des accouchements prévus à domicile en Essex

Une étude prospective a été faite auprès de 296 praticiens de l’Essex afin d’analyser les risques associés à l’accouchement à domicile. Les 202 femmes s’étant inscrites pour un accouchement à domicile ont été comparées à un groupe similaire de 185 femmes inscrites pour un accouchement à l’hôpital sous contrôle d’un médecin. La différence majeure en termes de résultats était un taux de 19% de déclenchement enregistré pour les accouchements à l’hôpital, alors qu’il était de 8% pour les accouchements à domicile. De même des taux plus élevés d’épisiotomie, de déchirure du second degré et de score Apgar à 7 ou inférieur ont été enregistrés dans le groupe des accouchements à l’hôpital. Aucun décès périnataux n’a été enregistré dans aucun des groupes.
Les résultats de cette étude n’ont donné aucune preuve d’augmentation des risques liés à l’accouchement à domicile, mais ont montré au contraire moins de complications que pour les femmes ayant accouché à l’hôpital.

Five year prospective survey of risk of booking for a home birth in Essex.

A prospective survey was undertaken in 26 practices in Essex to assess the risks associated with a home birth. The 202 women who were booked for a home confinement were compared with a similar group of 185 women who were booked for hospital delivery under consultant care. The principal difference in outcome was the induction rate of 19% in the hospital group compared with 8% in the group booked for delivery at home. A higher rate of episiotomy and second degree tears and more Apgar scores of 7 or below were found in those who were booked for hospital. There were no perinatal deaths in either group. The results of this study showed no evidence of an increased risk associated with home confinements but indicated that there were fewer problems than were encountered in the deliveries in mothers confined in hospital.
