Objectif
Identifier quels facteurs sont associés à la gravité de l’hémorragie post-partum, parmi : les caractéristiques des femmes et de leur accouchement, les composantes de la gestion de l’hémorragie initiale, et les caractéristiques d’organisation des maternités.

Méthode
Etude de cohorte (population-based cohort study) sur les femmes ayant eu une hémorragie post-partum due à une atonie utérine après un accouchement vaginal, dans 106 hôpitaux français, entre décembre 2004 et novembre 2006 (N=4550). L’hémorragie post-partum sévère est définie par une variation péripartum en hémoglobine de 4g/dL ou plus. Les facteurs associés de façon indépendantes à la sévérité de l’hémorragie post-partum ont été déterminés en utilisant un modèle d’analyse logistique multivariable (multivariable logistic model)

Résultats
Une hémorragie postpartum grave est survenue pour 952 femmes (20,9%). Chez les femmes qui ont eu une hémorragie post-partum, les facteurs indépendemment associés à la gravité étaient : primiparité ; antécédent d’hémorragie post-partum ; antécédent de césarienne ; m maturation cervicale (cervical ripening) ; travail prolongé ; épisiotomie ; retard dans la prise en charge initale de l’hémorragie post-partum. Ont été également associés à la gravité de l’hémorragie
1) l’administration d’ocytocine dans un délai supérieur à 10 minutes après le diagnostic d’hémorragie : pour les administrations après un délai de 10 à 20 minutes, la proportion d’hémorragie post-partum grave était de 24,6%, contre 20,5% (adjusted OR 1.38, 95% CI 1.03-1.85) ; pour les administration après un délai de 20 minutes, 31,8% contre 20,5% (adjusted OR 1.86, CI 1.45-2.38)
2) examen manuel de la cavité utériene après un délai de 20 minutes : proportion d’hémorragies post-partum grave 28,7%, contre 20,7% (adjusted OR 1.83, 95% CI 1.42-2.35)
3) demande d’assistance supplémentaire dans un délai de plus de 10 minutes : proportion d’hémorragies graves 29,8% contre 24,8 pour l’appel à un obstétricien ( adjusted OR 1.61, 95% CI 1.23-2.12) et 35.1% contre 29.9%, pour l’appel à un anesthésiste (adjusted OR 1.51, 95% CI 1.14-2.00)
4) accouchement dans un hopital public non universitaire
Pour les femmes ayant une hémorragie post-partum, on a trouvé à l’analgésie péridurale un facteur de protection contre les pertes sanguines graves.

Conclusion : des aspects du travail, de l’accouchement, de leur gestion ; du délai avant les soins initiaux ; et le lieu d’accouchement sont des facteurs de risque indépendants de pertes sanguines graves chez les femmes ayant une hémorragie post-partum causée par atonie utérine

Abstract (English)  :

OBJECTIVE:
To identify factors associated with severity of postpartum hemorrhage among characteristics of women and their delivery, the components of initial postpartum hemorrhage management, and the organizational characteristics of maternity units.

METHODS:
This population-based cohort study included women with postpartum hemorrhage due to uterine atony after vaginal delivery in 106 French hospitals between December 2004 and November 2006 (N=4,550). Severe postpartum hemorrhage was defined by a peripartum change in hemoglobin of 4 g/dL or more. A multivariable logistic model was used to identify factors independently associated with postpartum hemorrhage severity.

RESULTS:
Severe postpartum hemorrhage occurred in 952 women (20.9%). In women with postpartum hemorrhage, factors independently associated with severity were: primiparity; previous postpartum hemorrhage; previous cesarean delivery; cervical ripening; prolonged labor; and episiotomy; and delay in initial care for postpartum hemorrhage. Also associated with severity was 1) administration of oxytocin more than 10 minutes after postpartum hemorrhage diagnosis: 10-20 minutes after, proportion with severe postpartum hemorrhage 24.6% compared with 20.5%, adjusted OR 1.38, 95% CI 1.03-1.85; more than 20 minutes after, 31.8% compared with 20.5%, adjusted OR 1.86, CI 1.45-2.38; 2) manual examination of the uterine cavity more than 20 minutes after (proportion with severe postpartum hemorrhage 28.2% versus 20.7%, adjusted OR 1.83, 95% CI 1.42-2.35); 3) call for additional assistance more than 10 minutes after (proportion with severe postpartum hemorrhage 29.8% versus 24.8%, adjusted OR 1.61, 95% CI 1.23-2.12 for an obstetrician, and 35.1% compared with 29.9%, adjusted OR 1.51, 95% CI 1.14-2.00 for an anesthesiologist); 4) and delivery in a public non-university hospital. Epidural analgesia was found to be a protective factor against severe blood loss in women with postpartum hemorrhage.

CONCLUSION:
Aspects of labor, delivery, and their management; delay in initial care; and place of delivery are independent risk factors for severe blood loss in women with postpartum hemorrhage caused by atony.

The conditions under which postpartum haemorrhage becomes a serious bleeding are investigated. Independently identified factors: primiparity; history of postpartum hemorrhage; history of caesarean section; cervical ripening; prolonged work; episiotomy; delay in the initial management of postpartum haemorrhage (ie > 10 minutes or > 20 minutes between diagnosis of bleeding and oxytocin administration); uterine revision (“manual“ examination of the uterine cavity) > 20 minutes later; call of an obstetrician and/or anesthesia > 10 minutes later; delivery in public non-university hospital. Protective effect of the epidural, may be linked to speed of intervention.

Fatores identificados independentemente: primiparidade; história de hemorragia pós-parto; história de cesariana; amadurecimento cervical; trabalho prolongado; episiotomia; atraso no tratamento inicial da hemorragia pós-parto (ie > 10 minutos ou > 20 minutos entre o diagnóstico de sangramento e administração de ocitocina); revisão uterina (“manual“ exame da cavidade uterina) > 20 minutos após, chamada de um obstetra e/ou anestesia > 10 minutos após; parto em hospital público não universitário.
