RésuméObjectif
Afin de suivre les recommandations de pratiques cliniques (RPC) publiées en 2005 par le Collège national des gynécologues et obstétriciens de France (CNGOF) plaidant pour une politique restrictive de l’épisiotomie, notre service a entrepris un radical changement de ses pratiques en préconisant une restriction des indications de l’épisiotomie. Le but de cette étude est d’évaluer l’impact des RPC sur le taux d’épisiotomies et la répartition des lésions périnéales. Cette étude s’intègre dans une démarche d’amélioration continue des soins et correspond au point de départ d’un audit interne.
Patientes et méthodes
Il s’agit d’une étude d’évaluation non expérimentale type « avant-après » réalisée au sein d’une maternité universitaire de niveau 3 entre janvier 2004 et décembre 2009. L’ensemble des femmes ayant accouché par voie basse d’un enfant en présentation céphalique, à un terme supérieur ou égal à 37 SA a été inclus. Afin d’évaluer les conséquences de ce changement de pratique, il avait été demandé, d’une part, que l’indication de l’épisiotomie soit précisée dans le dossier obstétrical informatisé, et d’autre part, qu’un bilan global mensuel sous forme de tableaux de bord récapitulatifs soit effectué auprès de chaque membre de l’équipe de salle de naissances.
Résultats
Entre 2004 et 2009, nous avons observé une chute significative (p < 0,01) du taux global d’épisiotomies passant de 55,7 à 13,3 %. Cette diminution portait à la fois sur les primipares et sur les multipares, que la voie basse ait été spontanée ou assistée d’une extraction instrumentale (EI) (presque uniquement par ventouse). Une épisiotomie était réalisée dans 87 % des cas en 2004 contre 33,5 % des cas en 2009 (p < 0,01) lors d’une EI. Si le taux de lésions « mineures » sans conséquences fonctionnelles (déchirures vaginales ou périnéales simples, éraillures des petites lèvres) a augmenté de manière significative (27,1 % en 2004 vs 63,9 % en 2009, p < 0,01), le taux de lésions périnéales avec atteinte sphinctérienne (troisième et quatrième degré selon la classification anglo-saxonne) a légèrement baissé sans différence significative (1,3 % vs 1,1 %). En revanche, le taux de périnées intacts a significativement augmenté (17,6 % vs 21,7 %, p < 0,01), notamment dans les EI.
Discussion et conclusion
Nos résultats montrent une évolution favorable des taux d’épisiotomies sans accroissement des lésions périnéales sévères, en accord avec ceux des RPC mais encore loin des conclusions récentes de l’evidence-based medicine selon lesquelles la pratique de l’épisiotomie devrait être évitée. Dans notre étude, il semble que le retour des résultats aux professionnels ait un impact sur leur pratique, résultats confirmés par la littérature. Ainsi, après une relance de l’information fin 2008, nous avons assisté à une nouvelle décroissance du taux d’épisiotomies alors qu’il tendait à réaugmenter. Nos efforts doivent être poursuivis, notamment lors des EI, accouchements réalisés uniquement par les obstétriciens.
SummaryObjectives
Following the publication of the French Guidelines on episiotomy in 2005 by the French National College of Gynaecologists and Obstetricians (CNGOF), our unit decided to adopt a restrictive politics to replace the former liberal one. The goal of this study was to evaluate the impact of this modification of trend in the Unit of Gynaecology and Obstetrics of the teaching hospital from Caen. This was the start point of an internal audit with the objective of an enhancement of the care during delivery.
Patients and methods
It was a retrospective study concerning the period going from January, 2004 till December, 2009, measuring the impact of the guidelines on the episiotomy rates and perineal tears. The totality of the population of the women naturally delivered at a term superior or equal to 37 weeks of gestation and a cephalic presentation was included. To modify the practices, after diffusion of the guidelines by some obstetrical leaders, we imposed to notify the indication of the episiotomy in the computerized obstetrical files. Moreover, we published monthly screenboards with all the detailed results to all the practioners acting in the delivery room (obstetricians, midwives).
Results
Between 2004 and 2009, we observed a dramatic decline of the episiotomy rates from 55.7 to 13.3%. This trend was the same for primiparae and multiparae, as wall as for spontaneous or assisted delivery (mostly vacuum extraction in our unit). We noticed a slight increased in minor perineal tears without functional consequences. There was no statistically significant difference between severe perineal tear (1.3% vs. 1.1%). By contrary, the rate of intact perineum significantly raised (17.6% vs. 21.7%, p < 0.001), especially in vacuum extractions.
Discussion
Our results are in accordance with those of the national guidelines. However, our rate does not decrease in 2007 and 2008 (20%), contrary to some results obtained in the literature. Some reports mentioned the weak impact of the publication of national guidelines to modify the usual practice. However, we believe that retro-information to practitioners has a real impact on their daily current practice. Hence we insisted again on the importance of these national guidelines at the end of 2008 and we could demonstrate a real decline of the rate of episiotomy. Our efforts have to be continued, especially for instrumental delivery.
