Objective

Women who sustain third degree (3DT) anal sphincter tears are at increased risk of developing subsequent fecal incontinence and optimal treatment following immediate 3DT repair remains unknown, although pelvic floor exercises (PFE) are standard advice. This study aimed to compare postnatal continence scores and anal manomety in women recomended early home biofeedback physiotherapy (BFP) compared with women practising standard PFE following 3DT.
Study Design

Women were randomized following 3DT to early (commencing <30 days) postpartum home biofeedback physiotherapy (n=30) or conventional PFEs (n=90). Continence assessment was performed at 3 months postpartum in a dedicated Perineal Clinic.
Results

Neither resting anal pressures (39 +/− 13 mmHg BFP vs 43 +/− 17 mmHg PFE; (P=0.12) nor anal squeeze pressures (64 +/− 171 mmHg BFP vs 62+/− 23 PFE; P=0.68) were significantly different at 3 months postpartum between home BFP and PFE groups. Continence symptom scores and quality of life assessments were also similar between the groups.
Conclusion

Early postpartum BFP appears to add no advantage to standard PFE within 3 months following 3DT, although reduced patient compliance may have influenced results. Biofeedback physiotherapy should remain a first treatment option for women with ongoing or worsening continence symptom identified at follow-up after anal sphincter injury.
