“Objectif la recherche fait état d’une prévalence de 13 à 28% de maltraitances liées aux soins pour les femmes dans les pays scandinaves. L’étude a pour objet d’analyser le contenu des documents d’éthique destinés aux professionnels dans le domaine de l’obstétrique gynécologie, en Suède : dans quelle mesure les recommandations prennent-elles en compte les problèmes liés aux maltraitances
Conception recherche de données empiriques sur la maltraitance liée aux soins dans la littérature : recommandations pour les infirmiers, sages-femmes, médecins. Développement d’un cadre analytique. Analyse.
Résultats Grande variété de contenu des recommandations d’éthique sur la question des maltraitances liées aux soins. Les questions les plus souvent oubliées sont: la prise en compte de l’avis du patient, de son expérience potentielle de maltraitances, prendre en compte les déséquilibre du pouvoir dans les soins (?), abus sexuel, comment gérer les manquements éthiques d’un autre professionnel, relations entre les professionnels. Les deux recommandations les plus complètes sont: celles de la FIGO (fédération internationale de de gynécologie obstétrique) et celles de l’Internal confederation of midwives.
Conclusion Différents professionnels soignent les mêmes patients, mais leurs recommandations éthiques ne sont pas les mêmes. Pour que les recommandations éthique deviennent des outils de prévention de la maltraitance dans les soins, nous suggérons la mise à jour des recommandations de façon à intégrer la recherche empirique sur l’éthique.
L’existence seule de recommandations n’en garantit pas l’efficacité. Nous voulons initier le débat sur la fonction et l’utilisation des recommandations d’éthique en général, et de la maltraitance liée aux soins en particulier.
Les recommandations éthiques ne recouvrent cependant qu’une partie de l’éthique des soins. Nous envisageons une approche plus large de la prévention de la maltraitance : développer la recherche sur l’application de la vertu éthique (? “we envision a broader approach to the aim of preventing AHC, where research is encouraged on how a virtue ethics approach could be applied“)“

Abstract (English)  :

Objective
In obstetrical and gynaecological healthcare, patients often find themselves in a vulnerable position. Sensitive issues such as sexual and reproductive health are addressed and certain procedures can be experienced as abusive. According to research a lifetime prevalence of abuse in healthcare (AHC) can be assumed for 13–28% of female patients in the Nordic countries. In the present study we analyse the content of ethical documents for healthcare professionals within obstetrics and gynaecology in Sweden, in order to find out to what extent ethical guidelines consider issues that have shown to be related to the occurrence of AHC.

Study design
We searched the literature to find empirical data on AHC. Guidelines for nurses, midwives and physicians were selected. After developing an analytical framework based on the empirical data the content of the ethical guidelines was analysed.

Results
The various ethical guidelines for staff working within obstetrics and gynaecology differ distinctively from each other regarding their content of issues that are related to AHC. Issues that were mostly disregarded were: considering the patient’s perspective and the patients’ possible experience of violence, considering power imbalances within healthcare, sexual misconduct, how to deal with other professional’s ethical misconduct and how professionals relate to each other. We found the ethical guidelines of the International Federation of Gynecology and Obstetrics (FIGO) and of the International Confederation of Midwives to be those which contained most of the issues that have empirically shown to be important in regard to AHC.

Conclusion
While staff members from different professions may share responsibility for the same patient, their ethical guidelines vary considerably. To become a possible resource for prevention of AHC, we suggest that ethical guidelines in healthcare should be revised following empirical research on ethical conduct. As ethical guidelines cannot be effective by their existence only, we would like to initiate a discussion on the function and use of ethical guidelines in general and regarding AHC in particular. Being aware that ethical guidelines are only a part of ethics in healthcare, however, we envision a broader approach to the aim of preventing AHC, where research is encouraged on how a virtue ethics approach could be applied.

Etude qui s’intéresse à 14 recommandations d’éthique destinées aux professionnels de santé en général et en santé des femmes en particulier : médecins/obstétriciens, infirmiers, sages-femmes ; Suède et monde. La question est : dans quelle mesure ces recommandations prennent-elles en compte les questions de maltraitance liées aux soins. 14 questions.
