Objectifs. En Tanzanie, il y a une forte mortalité maternelle et une faible proportion d’accouchement dans des établissements de santé, en dépit des efforts pour réduire les obstacles à leur utilisation. Le manque de respect et les maltraitances pendant l’accouchement n’ont pas encore étudiés dans leur aspects de contributeurs à la satisfaction, ou dissuasif vis-à-vis des accouchements dans une structure de soins. Nous avons évalué l’association entre le fait de rapporter un traitement non respectueux pendant l’accouchement, et : la satisfaction vis-à-vis de l’accouchement; la qualité ses soins perçue; et l’intention de retourner accoucher dans le même établissement.
Méthodes. Entretiens avec questionnaire structurés, conduits dans une région de Tanzanie (Tanga) auprès de femmes sortant de deux hôpitaux après leur accouchement. Le non-respect et les maltraitances ont été évalué en interrogeant les femmes sur des événements spécifiques pendant l’accouchement. Des modèles de régression logistique ont été utilisés pour évaluer l’association entre le non-respect et maltraitance et (1) la satisfaction vis-à-vis de l’accouchement (2) la qualité de soins perçue et (3) l’intention de retourner accoucher dans la même maternité; avec contrôle des facteurs de confusion.
Résultats. 1388 femmes ont participé à l’enquête (taux de réponse 67%). Le non-respect et la maltraitance pendant l’accouchement était associé à une plus faible satisfaction vis-à-vis de l’accouchement (OR 0.26, 95 % CI 0.19–0.36) et une réduction des chances de noter la qualité des soins comme excellente/très bonne (OR 0.55, 95 % CI 0.35–0.85). Parmi les femmes qui prévoient d’avoir d’autres enfants (N = 766), celles qui ont vécu le non-respect ou la maltraitance étaient deux fois moins susceptibles d’avoir l’intention de revenir accoucher au même endroit (OR 0.51, 95 % CI 0.32–0.82).
Conclusions. Notre étude souligne que le non-respect et la maltraitance pendant l’accouchement sont des importants facteurs de diminution de la confiance des femmes dans les établissements de santé. L’amélioration des relations inter-personnelles dans les soins (“interpersonal care“) doit faire partie intégrante les améliorations de la qualité en santé maternelle.

Abstract (English)  :

Objectives In Tanzania, maternal mortality is high and coverage with health facility delivery low, despite efforts to reduce barriers to utilization. Disrespect and abuse during childbirth has not been explored as a contributor to delivery satisfaction or as a deterrent to institutional delivery. We assessed the association between reported disrespectful treatment during childbirth and delivery satisfaction, perceived quality of care, and intention to deliver at the same facility in the future. Methods Interviews using a structured questionnaire were conducted in Tanga Region, Tanzania with women on discharge from delivery at two hospitals. Disrespect and abuse was measured by asking women about specific disrespectful events during childbirth. Multivariable logistic regression models were used to assess the association between disrespect/abuse and (1) satisfaction with delivery, (2) perceived quality of care for delivery, and (3) intent to use the same facility for a future delivery, controlling for confounders. Results 1388 women participated in the survey (67 % response rate). Disrespect/abuse during childbirth was associated with lower satisfaction with delivery (OR 0.26, 95 % CI 0.19–0.36) and reduced likelihood of rating perceived quality of care as excellent/very good (OR 0.55, 95 % CI 0.35–0.85). Of women who planned to have more children (N = 766), those who experienced disrespect/abuse were half as likely to plan to deliver again at the same facility (OR 0.51, 95 % CI 0.32–0.82). Conclusions Our study highlights disrespectful and abusive treatment during childbirth as an important factor in reducing women’s confidence in health facilities. Improving interpersonal care must be an integral part of quality improvement in maternal health.
