Objectifs : identifier les caractéristiques des patients qui gardent le silence vis-à-vis du système de soins suite à des expériences de comportement abusif ou éthiquement injustifié des personnels de santé.

Conception : étude transversale utilisant le questionnaire Ethical Principles in Health Care (principes d’éthique de soins)

Lieu : clinique pour femmes dans le sud de la Suède

Les critères de sélection des participants étaient : patientes (femmes) ayant des rendez-vous consécutifs en consultation externe, âgées de 18 ans et plus, parlant en comprenant le suédois, et ayant une adresse connue. 534 femmes (60%) ont répondu au questionnaire. Parmi elles, 293 ont été incluses pour l’étude.

Mesure principale : combien de fois les répondantes ont gardé le silence vis-à-vis du système de soins, par rapport au nombre de fois où elles ont parlé.

Résultats
Une association a été trouvée entre le fait que les patientes gardent le silence, et un jeune âge, ainsi qu’une faible connaissance (auto-évaluée) des droits des patients. Les deux variables ont un effet indépendant dans un modèle multivarié. Pas d’association trouvé avec le statut social, le pays de naissance, la santé ou d’autres maltraitances.

Conclusion.
Ces résultats ouvrent la voie à la conception d’interventions destinées à stimuler l’expression des patients,“and open up the clinical climate, for which the responsibility lies in the hands of staff“. D’autres recherches sont nécessaires.

Abstract (English)  :

Objectives To identify which patient characteristics are associated with silence towards the healthcare system after experiences of abusive or ethically wrongful transgressive behaviour by healthcare staff. Design Cross-sectional questionnaire study using the Transgressions of Ethical Principles in Health Care Questionnaire.

Setting
A women’s clinic in the south of Sweden.

Participants
Selection criteria were: consecutive female patients coming for an outpatient appointment, ≥18-year-old, with the ability to speak and understand the Swedish language, and a known address. Questionnaires were answered by 534 women (60%) who had visited the clinic, of which 293 were included in the present study sample.

Primary outcome measure
How many times the respondent remained silent towards the healthcare system relative to the number of times the respondent spoke up.

Results
Associations were found between patients’ silence towards the healthcare system and young age as well as lower self-rated knowledge of patient rights. Both variables showed independent effects on patients’ silence in a multivariate model. No associations were found with social status, country of birth, health or other abuse.

Conclusions
The results offer opportunities for designing interventions to stimulate patients to speak up and open up the clinical climate, for which the responsibility lies in the hands of staff; but more research is needed

Chez les patientes rapportant des maltraitances liées aux soins, le fait de garder le silence vis-à-vis du système de soins est associé à un jeune âge, et à des patients qui auto-évaluent leurs connaissances sur les droits des patients comme faibles. Pas d’influence du statut social, du pays de naissance, de l’état de santé ou d’autres maltraitances.
