Contexte. Dans une étude précédente, conduite à l’aide du questionnaire NorVold Abuse questionnaire [ou NordAQ] , nous avons mis en évidence que parmi les patientes confrontées, à l’âge adulte, à la maltraitance liée aux soins, un tiers avaient un historique de maltraitance émotionnelle, physique et/ou sexuelle dans leur enfance (“revictimisées“). Cependant la majorité des victimes de maltraitance liée aux soins sont des “nouvelles victimes“, sans historiques. Il pourrait donc y avoir d’autres facteurs associés aux maltraitances liées aux soins à l’âge adulte. L’objet de cette étude est de cartographier la prévalence des maltraitances liées aux soins et des variables associées parmi les patientes nouvelles victimes et “revictimisées“ Méthodes. Questionnaire cross-sectionnel dans 5 établissements gynécologiques dans des pays Scandinaves. Les associations entre maltraitance liée aux soins à l’âge adulte et d’autres variables ont été testées dans un modèle multivarié, dans 4 groupes de femmes : EPSA (historique de maltraitance émotionnelle, physique et/ou sexuelle), EPSA dans l’enfance, EPSA dans l’enfance ET à l’âge adulte, pas d’EPSA Résultats : le taux de réponse a été de 77% (n=3641). La prévalence des maltraitances liées aux soins à n’importe quelle période de la vie est de 13-28%. Niveau d’étude supérieur, plaintes physiques, symptôme de stress post-traumatique, problèmes de sommeil sont associés à la maltraitance liée aux soins à l’âge adulte chez les femmes sans EPSA (nouvelles victimes). Un état de santé auto-évalué comme faible est fortement associé à la maltraitance liée aux soins à l’âge adulte, dans les 3 groupes de femmes avec historique de maltraitance Conclusion : Les patientes en gynécologie dans les pays scandinaves rapportent des maltraitances liées aux soins, mais ce phénomène n’est pas correctement exploré ni compris. Les facteurs associés aux maltraitances liées aux soins sont un haut niveau d’étude et un mauvais état de santé auto-évalué

Abstract (English)  :

Background. In an earlier Swedish study conducted with The NorVold Abuse Questionnaire, we found that one-third of female patients who had experienced adult abuse in health care (AAHC) had a background of childhood emotional, physical and/or sexual abuse (EPSA) (’revictimised’). But since the majority of women with AAHC were ’new victims’ without such a background, there might be other factors associated with AAHC. The present study aimed to map prevalence of abuse in health care (AHC), and associated variables among new victims and revictimised patients. Methods. We carried out a cross-sectional questionnaire study at 5 Nordic gynecological clinics. Associations between AAHC and other variables were tested in a multivariate model in 4 groups of women with adult EPSA, childhood EPSA, childhood and adult EPSA, and no EPSA. Results. The response rate was 77% (n=3,641). The overall prevalence of any lifetime experience of AHC was 13-28%. High educational level, physical complaints, post-traumatic stress symptoms, and sleeping problems were associated with AAHC in women without EPSA (new victims). Poor self-rated health was strongly associated with AAHC in the 3 groups of women with EPSA. Conclusion. AHC is commonly reported by gynecological patients in the Nordic countries, but not yet properly explored or understood. The most important factors associated with AAHC were high educational level and poor self-rated health.

On sait par d’autres études qu’un tiers d’entre elles avaient déjà subi des maltraitances émotionnelles, physiques et/ou sexuelles dans l’enfance. Cette étude cherche à comprendre les autres facteurs. Résultats: les “nouvelles victimes“ (qui n’avaient pas subi de maltraitance dans l’enfance) ont plus souvent un niveau d’éducation supérieur; parmi les femmes “re-victimisées (ayant subi des maltraitances dans l’enfance), elles sont beaucoup plus nombreuses à s’auto-évaluer un mauvais état de santé.
