Objectif
Comprendre la perception de la maltraitance liée aux soins de la part de personnels de santé après une intervention sous forme d’ateliers théâtre de mise en situation; comparaison avec des entretiens préalables à l’intervention, et entretiens avec des patients (hommes et femmes). La maltraitance liée aux soins est décrite comme un défaut de prise en compte du point de vue du patient

Conception - setting
Étude qualitative de suivi d’entretiens. Clinique gynécologique, Suède (women’s clinic) Participants Lors de l’étude de pré-intervention, 21 entretiens ont été réalisés parmi les membres du personnel. Parmi elles, 14 personnes ont participé à l’intervention et ont été éligibles à l’étude de suivi. Ils sont 4 à avoir refusé l’invitation, l’étude de suivi a donc porté sur 10 personnes.

Intervention
Entre janvier 2008 et janvier 2009, tous les membres du personnel (N=136) ont été conviés à des ateliers théâtre (“Forum Play workshop“). 74 d’entre eux ont participé à au moins un des 17 ateliers d’une demi-journée. La mesure principale était la perception des maltraitances liées aux soins par les personnels

Résultats
La catégorie principale ‘a summoning stone in the shoe’ (un caillou dans la chaussure) est construite à partir de 5 catégories: “déshumanisation du patient“, “inacceptable: il faut agir“, “Ubiquitous“ (omniprésent, universel), “non intentionnel“, “relatif“. La pièce de théâtre met en avant les possibilités d’agir, même dans des situations où cela semble “impossible“, et montre que le caractère tabou des maltraitances liées aux soins a été modifié dans l’établissement. La comparaison avec l’étude de pré-intervention montre une augmentation de la conscience des maltraitances liées aux soins, plus d’exemple concrets de la maltraitances, une empathie plus importante envers les patients, et moins d’explications, de justification et de banalisation des maltraitances.

Conclusion
Dans cette étude de suivi, les soignants ont une perception des maltraitances liées aux soins qui s’est rapprochée de celle des patients. Par rapport aux entretiens de pré-intervention, les personnels ont montré une plus grande volonté non seulement de reconnaître les maltraitances, mais aussi de prendre la responsabilité d’agir pour y mettre fin et les prévenir. Cette attitude pourrait s’expliquer par le fait que les ateliers théâtre montrent qu’il y a des possibilités d’action, et que le tabou qui pèse sur les maltraitances a été brisé dans cet établissement

Abstract (English)  :

Objective
The study aim was to apprehend staff’s perception of abuse in healthcare (AHC) after an intervention based on ‘Forum Play’, and make comparisons to preintervention interviews and interviews with male and female patients. AHC can be described as a failing encounter from the patient’s perspective. Design Qualitative interview follow-up study.

Setting
A Swedish Women’s Clinic.

Participants
In a preintervention study 21 staff members were interviewed. Eligible for the follow-up study were 14 informants who had participated in the intervention. Four declined participation leaving ten informants for this study. Intervention During January 2008–January 2009, all staff members (N=136) were invited to participate in Forum Play workshops. Seventy-four participants took part in at least 1 of the 17 half-day workshops. Primary outcome measures Staffs perception of AHC.

Results
The core category, ‘a summoning stone in the shoe’, was constructed of five categories: ‘Dehumanising the patient’, ‘Unacceptable: you are bound to act!’, ‘Ubiquitous’, ‘Unintentional’ and ‘Relative’. Forum Play had demonstrated possibilities to act even in seemingly ‘impossible’ situations, and that the taboo status of AHC was altered at the clinic. When our results were compared to those in the preintervention study, we found an increased awareness about AHC, more concrete examples of AHC, a stronger empathy for patients, and fewer explanations, justifications and trivialisations of AHC.

Conclusion
In this follow-up study staff’s perception of AHC was closer to the patient’s perspective. Compared to the preintervention interviews staff showed a greater willingness not only to acknowledge AHC, but also to take on a responsibility to act in order to stop or prevent AHC. Explanations for this stance could be that Forum Play had showed staff that there were possibilities to act, and that the taboo status of AHC had been broken at the clinic.

Etude qui nous semble prometteuse sur la possibilité de faire évoluer la perception qu’ont les personnels soignants de la maltraitance liée aux soins. Il s’agissait d’ateliers théâtre “theatre forum“ qui mettent les participants en situation qu’ils ont eux-mêmes vécue, et dans laquelle ils ont été les témoins impuissants d’une maltraitance perçue par le patient et ne sont pas intervenus car ils pensaient qu’on ne pouvait rien y faire. La pièce est rejouée jusqu’à ce que tous les participants comprennent la nature du conflit moral, et quelles sont les personnes qui en souffrent. Puis elle est rejouée à nouveau, et là les participants sont encouragés à intervenir en prenant le rôle de la personne qui souffre du dilemme, de manière à influencer la situation. Etude qui nous semble prometteuse sur la possibilité de faire évoluer la perception qu’ont les personnels soignants de la maltraitance liée aux soins. Il s’agissait d’ateliers théâtre qui mettent les participants en situation qu’ils ont eux-mêmes vécue, et dans laquelle ils ont été le témoin impuissants d’une maltraitance perçue par le patient et ne sont pas intervenus car ils pensaient qu’on ne pouvait rien y faire. La pièce est rejouée jusqu’à ce que tous les participants comprennent la nature du conflit moral, et quelles sont les personnes qui en souffrent. Puis elle est rejouée à nouveau, et là les participants sont encouragés à intervenir en prenant le rôle de la personne qui souffre du conflit moral, de manière à influer sur la situation. Les chercheurs ont mené des entretiens auprès des participants, avant et après ces ateliers théâtre. Après les ateliers, les soignants avaient une perception des maltraitances liées aux soins plus proche de celle des patients; une plus grande volonté à reconnaître les maltraitances; mais aussi, une plus grande volonté d’agir pour prévenir les maltraitances ou pour y mettre fin.
