Contexte
Chaque femme a le droit de recevoir des soins dignes et respectueux lors de l’accouchement. Des preuves récentes ont montré que dans le monde, de nombreuses femmes subissaient des mauvais traitements pendant le travail et l’accouchement, ce qui pouvait constituer un obstacle important pour les femmes qui accouchent dans des établissements et contribuer aux mauvaises expériences d’accouchement et aux résultats défavorables pour les femmes et les nouveau-nés. Cependant, il n’existe pas de consensus clair sur la définition et la mesure des mauvais traitements infligés aux femmes pendant l’accouchement. Nous proposons d’utiliser une méthodologie d’étude en deux phases et méthodes mixtes dans quatre pays pour combler ces lacunes dans la recherche. Ce protocole décrit les activités de recherche qualitative de la phase 1.

Méthodes/Conception
Nous utiliserons des méthodologies de recherche qualitative auprès des femmes, des prestataires de soins de santé et des administrateurs dans les zones de chalandise des établissements de deux établissements de santé dans chaque pays : Ghana, Guinée, Myanmar et Nigéria. Des entretiens approfondis (IDI) et des discussions de groupe (FGD) seront conduits auprès de femmes en âge de procréer (15 à 49 ans) afin d’explorer leurs perceptions et leurs expériences en matière de soins à l’accouchement en établissement, axées sur la manière dont elles ont été traitées par les agents de santé. et les facteurs perçus affectant la façon dont ils ont été traités. Des IDI seront également organisés avec des prestataires de soins de différents cadres (infirmières, sages-femmes, médecins, obstétriciens spécialisés, etc.) et des administrateurs d’institutions travaillant dans les établissements sélectionnés pour explorer les perceptions et les expériences des prestataires de soins de santé en matière de comment sont traités le personnel, les collègues et les superviseurs. Les enregistrements audio seront transcrits et traduits en anglais. Les données textuelles seront analysées à l’aide d’une approche-cadre thématique et comporteront deux niveaux d’analyse : (1) organisation d’ateliers d’analyse locaux avec les assistants de recherche de chaque pays ; et (2) le codage ligne par ligne pour développer un cadre thématique et un schéma de codage.

Discussion : Cette étude remplit plusieurs rôles. Il fournira une compréhension approfondie de la manière dont les femmes sont traitées pendant l’accouchement dans quatre pays et des facteurs perçus associés à ces mauvais traitements. Il fournira également des données sur où et comment une intervention pourrait être développée pour réduire les mauvais traitements et promouvoir des soins respectueux. Les résultats de cette étude contribueront à la mise au point d’outils permettant de mesurer la prévalence de la maltraitance envers les femmes lors de l’accouchement dans un établissement.

Abstract (English)  :

Background
Every woman has the right to dignified, respectful care during childbirth. Recent evidence has demonstrated that globally many women experience mistreatment during labour and childbirth in health facilities, which can pose a significant barrier to women attending facilities for delivery and can contribute to poor birth experiences and adverse outcomes for women and newborns. However there is no clear consensus on how mistreatment of women during childbirth in facilities is defined and measured. We propose using a two-phased, mixed-methods study design in four countries to address these research gaps. This protocol describes the Phase 1 qualitative research activities.

Methods/Design
We will employ qualitative research methodologies among women, healthcare providers and administrators in the facility catchment areas of two health facilities in each country: Ghana, Guinea, Myanmar and Nigeria. In-depth interviews (IDIs) and focus group discussions (FGDs) will be conducted among women of reproductive age (15–49 years) to explore their perceptions and experiences of facility-based childbirth care, focused on how they were treated by healthcare workers and perceived factors affecting how they were treated. IDIs will also be conducted with healthcare providers of different cadres (e.g.: nurses, midwives, medical officers, specialist obstetricians) and facility
administrators working in the selected facilities to explore healthcare providers’ perceptions and experiences of facility-based childbirth care and how staff are treated, colleagues and supervisors. Audio recordings will be transcribed and translated to English. Textual data will be analysed using a thematic framework approach and will consist of two levels of analysis: (1) conduct of local analysis workshops with the research assistants in each country; and (2) line-by-line coding to develop a thematic framework and coding scheme.

Discussion: This study serves several roles. It will provide an in-depth understanding of how women are treated during childbirth in four countries and perceived factors associated with this mistreatment. It will also provide data on where and how an intervention could be developed to reduce mistreatment and promote respectful care. The findings from this study will contribute to the development of tools to measure the prevalence of mistreatment of women during
facility-based childbirth.
