Déterminer la fréquence de l’incontinence anale après périnée complet et analyser les facteurs pronostiques. Étude rétrospective unicentrique incluant les patientes ayant présenté un périnée complet entre janvier 2006 et décembre 2012 dans une maternité de type 3. Les patientes ont été interrogées et examinées sur le plan gynécologique et proctologique à 2 mois du post-partum. L’incontinence anale était définie comme la présence d’au moins un symptôme : incontinence aux gaz, incontinence aux selles, impériosités aux selles. Parmi les 17 110 patientes ayant accouché par voie basse, 134 (0,8 %) ont présenté un périnée complet. Les données étaient disponibles pour 110 patientes. Parmi celles-ci, 50 femmes (45,5 %) présentaient au moins un symptôme d’incontinence anale à 2 mois et 8 (7,3 %) présentaient une incontinence aux selles. L’âge maternel et la durée à dilatation complète étaient significativement associés à la survenue d’une incontinence anale après périnée complet. Le degré d’atteinte des fibres sphinctériennes (IIIa, b, c, IV) lors de l’accouchement n’était pas un facteur associé au risque d’incontinence anale à 2 mois du post-partum. En dehors de l’âge maternel et de la durée à dilatation complète, aucun facteur de risque d’incontinence anale après périnée complet n’a été identifié dans notre série. La prévalence de l’incontinence anale à 2 mois du post-partum est élevée quel que soit le degré d’atteinte lors de l’accouchement. Nos résultats soulignent l’importance de ne pas sous-diagnostiquer les lésions sphinctériennes quel que soit leur stade.
To determine prevalence of short-term postpartum anal incontinence after obstetrical anal sphincter injury and prognostic factors. Retrospective study including every patient with an obstetrical anal sphincter injury between January 2006 and December 2012 in one tertiary maternity unit. Patients were interviewed and examined at 2-month postpartum. Anal incontinence was defined by the presence of at least one of the following symptoms: flatus incontinence, faecal incontinence and faecal urgency. Among 17,110 patients who delivered vaginally during period study, 134 (0.8%) presented an anal sphincter injury. Postpartum obstetrical data were available for 110 of them. Among those patients, 50 women (45.5%) had at least one symptom of anal incontinence at 2-month postpartum and 8 (7.3%) had faecal incontinence. Only maternal age and second stage duration were significantly associated with anal incontinence after obstetrical anal sphincter injury. The degree of sphincter damage at delivery (IIIa, b, c, IV) was not associated with the risk of anal incontinence at 2-month postpartum. Maternal age and second stage duration were the only risk factor for anal incontinence after obstetrical anal sphincter injury in this study. High prevalence of anal incontinence at 2-month postpartum of obstetrical anal sphincter injury is observed no matter what is the degree of anal sphincter damage. Our results highlight the importance to diagnose all obstetrical anal sphincter injuries whatever the degree of damage.
