Contexte et objectifs : Comparer l’efficacité et la sécurité de la dinoprostone, du misoprostol et du ballonnet de Cook comme déclencheurs du travail chez les femmes présentant un fœtus en retard de croissance intra utérin (RCIU) à terme.
Méthode : Etude de cohorte rétrospective sur dossiers de femmes diagnostiquées avec un fœtus RCIU à terme à l’hôpital Reina Sofia, Cordoba, Espagne, de Janvier 2014 à Décembre 2015. L’enregistrement des caractéristiques de base et la méthode de déclenchement a été faite. Le résultat principal était le temps entre le déclenchement et l’accouchement. Les issues obstétricales et périnatales ont également été récoltées.
Résultats : Un total de 99 femmes ont été diagnostiquées avec un RCIU dans ladite période. Parmi elles, 21 femmes ont été déclenchées au dinoprostone (groupe dinoprostone DG), 20 au misoprostol (MG), et 58 au ballonnet de Cook (CG). Les groupes étaient homogènes concernant les scores de Bishop et la parité pré-déclenchement. Le CG (groupe Ballonnet de Cook) ont nécessité plus de temps de travail (24,36 vs 19,23 heures ; p<0.01) ainsi qu’un plus grand recours à l’ocytocine pour diriger le travail (6.75 mUI vs 1.24 mUI ; p<0.01) comparé au DG. De plus, le CG nécessitait plus d’ocytocine que le MG, 6.75 vs 2.37 mUI (p<0.001). Les taux de césariennes étaient à 5, 14.9 et 17.3% dans les groupes MG, DG et CG respectifs. Aucune différence n’a été observée concernant les taux de tachysystole utérine, un statut fœtal non rassurant, et les évènements indésirables néonataux.
Conclusions : Les prostaglandines ont été plus efficaces que le ballonnet de Cook afin de déclencher le travail et atteindre un accouchement par voie basse dans cet échantillon de femmes à terme dont le fœtus souffrait d’un retard de croissance intra utérin, avec des profils de sécurité similaires.

Abstract (English)  :

Background and objectives: To compare effectiveness and safety of dinoprostone, misoprostol and Cook’s balloon as labour-inducing agents in women with intrauterine growth restriction (IUGR) at term. Methods: Retrospective cohort chart review of women diagnosed with foetal growth restriction at term in Reina Sofia Hospital, Cordoba, Spain from January 2014 to December 2015. Registration of baseline characteristics and method of induction was made. The main outcome was time from induction to delivery. Obstetric and perinatal outcomes were also collected. Results: A total of 99 women were diagnosed with IUGR in the mentioned period. Of them, 21 women were induced with dinoprostone [dinoprostone group (DG)], 20 with misoprostol (MG) and in 58 with Cook’s balloon (CG). Groups were homogeneous regarding pre-induction Bishop score and parity. The CG required more time (24.36 vs. 19.23 h; p = 0.02) and more oxytocin dose for conduction of labour from induction to delivery (6.75 vs. 1.24 mUI; p < 0.01) than DG. Moreover, the CG also needed more oxytocin than MG, 6.75 vs. 2.37 mUI (p < 0.001). Caesarean rate was 5, 14.9 and 17.3% in MG, DG and CG, respectively. No differences were observed in rates of uterine tachysystole, non-reassuring foetal status and neonatal adverse events. Interpretation and conclusions: Prostaglandins were more effective than Cook’s balloon to induce labour and achieve vaginal birth in this sample of women with IUGR at term, with a similar safety profile. © 2017, Springer-Verlag GmbH Germany.
