Alors que le nombre de femmes planifiant un accouchement à domicile dans les pays développés reste bas, ce nombre a augmenté durant cette dernière décennie aux Etats-Unis, et des preuves montrent que plus de femmes choisiraient cette option si elle était vraiment disponible. Les taux d’accouchement à domicile se situent entre 0,1% en Suède et 20% aux Pays-Bas, où l’accouchement à domicile a toujours été intégré dans le système de santé/maternité.
Les bénéfices de l’accouchement à domicile incluent : des taux inférieurs de morbidité maternelle, tels que l’hémorragie post partum et les déchirures périnéales, et moins d’interventions comme l’épisiotomie, l’instrumentation, ou la césarienne.
Les femmes ayant planifié un accouchement à domicile ont de meilleurs taux de satisfaction, liés à cet environnement plus confortable qu’est la maison, et la sensation de contrôler d’avantage la situation. Alors que les issues maternelles liées à l’accouchement à domicile ont toujours été positives dans la littérature, les issues néonatales sont plus variables. Alors que la majorité des études comparant les accouchements à domicile aux accouchements hospitaliers ne retrouvent aucune différence concernant la mortalité fœtale intrapartum, la mortalité néonatale, les scores d’Apgar bas, ou une admission en service de soins néonataux, un rapport aux Etats-Unis ainsi qu’une méta analyses indiquent de moins bonnes issues néonatales associées à l’accouchement à domicile.
De nombreux défis sont associés à la conception de recherches axées sur l’accouchement à domicile, en partie à cause de la non faisabilité d’une étude randomisée. Ce rapport passera en revue les recherches actuelles publiées entre 2004 et 2014 concernant les issues maternelles et néonatales des accouchements à domicile, et discuter des forces, des limites et des opportunités concernant l’accouchement à domicile.

Abstract (English)  :

While the number of women in developed countries who plan a home birth is low, the number has increased over the past decade in the US, and there is evidence that more women would choose this option if it were readily available. Rates of planned home birth range from 0.1% in Sweden to 20% in the Netherlands, where home birth has always been an integrated part of the maternity system.
Benefits of planned home birth include lower rates of maternal morbidity, such as postpartum hemorrhage, and perineal lacerations, and lower rates of interventions such as episiotomy, instrumental vaginal birth, and cesarean birth.

Women who have a planned home birth have high rates of satisfaction related to home being a more comfortable environment and feeling more in control of the experience. While maternal outcomes related to planned birth at home have been consistently positive within the literature, reported neonatal outcomes during planned home birth are more variable. While the majority of investigations of planned home birth compared with hospital birth have found no difference in intrapartum fetal deaths, neonatal deaths, low Apgar scores, or admission to the neonatal intensive care unit, there have been reports in the US, as well as a meta-analysis, that indicated more adverse neonatal outcomes associated with home birth.

There are multiple challenges associated with research designs focused on planned home birth, in part because conducting randomized controlled trials is not feasible. This report will review current research studies published between 2004 and 2014 related to maternal and neonatal outcomes of planned home birth, and discuss strengths, limitations, and opportunities regarding planned home birth.
