Aux Etats Unis, environ 35 000 naissances (0,9%) par an se déroulent à domicile. Approximativement ¼ de ces accouchements n’étaient pas planifiés à domicile, ou non-assistés. Bien que le Collège Américain des Gynécologues Obstétriciens pensent que les hôpitaux et les centres de naissances accrédités soient les lieux de naissances les plus sûrs, chaque femme a le droit de prendre une décision médicalement informée concernant son accouchement.
Le plus important, les femmes doivent être informées que de nombreux facteurs sont nécessaires à la réduction des taux de mortalité périnataux et à obtenir des issues favorables en cas d’accouchement à domicile. Ces facteurs incluent une sélection appropriée des candidats à un accouchement à domicile ; la disponibilité d’une infirmière-sage-femme certifiée, une sage-femme certifiée ou encore une sage-femme dont les études et le diplôme sont conformes aux Standards globaux des études de sage-femme de la Confédération Internationale des Sages-femmes (ICM), ou enfin un médecin pratiquant l’obstétrique intégré dans un système de santé régulé ; un accès facile aux consultations ; une possibilité de transport sûr et rapide jusqu’aux hôpitaux proches.
Le Comité de Pratique Obstétricale considère une mauvaise présentation fœtale, une grossesse multiple, ainsi qu’une précédente césarienne comme étant des contres indications absolues à une planification d’accouchement à domicile.

Abstract (English)  :

In the United States, approximately 35,000 births (0.9%) per year occur in the home. Approximately one fourth of these births are unplanned or unattended.
Although the American College of Obstetricians and Gynecologists believes that hospitals and accredited birth centers are the safest settings for birth, each woman has the right to make a medically informed decision about delivery.
Importantly, women should be informed that several factors are critical to reducing perinatal mortality rates and achieving favorable home birth outcomes. These factors include the appropriate selection of candidates for home birth; the availability of a certified nurse–midwife, certified midwife or midwife whose education and licensure meet International Confederation of Midwives’ Global Standards for Midwifery Education, or physician practicing obstetrics within an integrated and regulated health system; ready access to consultation; and access to safe and timely transport to nearby hospitals.
The Committee on Obstetric Practice considers fetal malpresentation, multiple gestation, or prior cesarean delivery to be an absolute contraindication to planned home birth.
