Accoucher par le siège, un choix qui doit être bien préparé

Quelques mois après la publication, en 2000, de l’article d’Hannah (1) sur les risques de l’accouchement par le siège, le taux de césarienne programmée pour siège a explosé dans de nombreux pays. Ainsi, il est passé de 24 % à 60 % aux Pays-Bas avec, pour conséquence, une perte d’expertise dans la prise en charge de la tentative de voie basse. Une maternité néerlandaise a décidé de vérifier si le mode d’information donné aux patientes avait une influence sur leur choix de la voie d’accouchement.

Dans cet établissement, il est proposé, à toutes les patientes ayant une présentation du siège, une version par manœuvre externe, en dehors bien sûr de celles qui ont une contre-indication à la voie basse, des grossesses multiples et des utérus bicicatriciels. En cas d’échec de la version, le mode d’accouchement est discuté selon 3 façons différentes. L’une débute par la recherche des souhaits de la patiente, par une sage-femme, suivie de l’exposition des bénéfices de l’un ou l’autre des modes d’accouchement avec un focus sur les situations à bas risque et l’évaluation des chances de succès de la voie basse. En cas de refus, après analyse des barrières, la patiente rencontre l’obstétricien qui réexplique les options, et le choix final s’effectue collégialement. La deuxième façon consiste essentiellement en une présentation des résultats de l’étude d’Hannah. La troisième manière d’aborder le sujet, peu structurée, dépend des préférences de l’obstétricien. A noter que cette dernière est historiquement la plus ancienne à avoir été employée et que parmi les deux autres, le choix se fait en fonction du moment où doit se produire la version et la disponibilité des intervenants.

Trois modalités de conseil comparées

Les dossiers de 309 femmes ayant accouché d’un siège entre 2007 et 2015 ont été examinés. Dans le premier groupe (1ère technique de conseil, n=155), 58 % avaient opté pour la voie basse et parmi elles 72 % ont effectivement accouché par voie basse ; dans le 2e (2e technique de conseil, n=82) et le 3e groupe (n=72), elles n’étaient plus que, respectivement, 15 % et 24 % à la tenter avec 50 % et 47 % de succès.

Le choix du mode d’accouchement est donc significativement lié au mode d’information délivré à la patiente. Une prise en charge personnalisée semble, dans ce domaine, donner des résultats très satisfaisants.

Abstract (English)  :

Objective

Women in the Netherlands, with a fetus in breech presentation, are thoroughly counseled to make an informed choice for the mode of delivery. The aim of this study was to assess the influence of counseling techniques on women’s choices for the mode of delivery and subsequently to compare fetal and maternal outcomes of vaginal breech birth versus planned caesarean section.

Study design

We performed an observational prospective study. Data on breech deliveries were prospectively collected. We used ANOVA to identify variables influencing women’s choice for the mode of delivery.

Setting

The obstetric department of the Red Cross Hospital in Beverwijk, the Netherlands.

Participants

Women with a singleton gestation (>37 + 0 weeks) and a fetus in breech presentation were included.

Measurements and findings

Between January 2007 and December 2015 364 women were included. Counseling technique (p =<0.001) and maternal education (p = 0.046) were significantly associated with the choice of mode of delivery. Of all included women 33% (N = 119) opted for a vaginal breech delivery and 52% (N = 190) opted for a planned cesarean section. 15% (N = 55) were unexpected breeches. Of the planned vaginal delivery group 66% (N = 79) delivered vaginal, whereas 99.5% (N = 189) of the women in the planned cesarean section group underwent a planned cesarean section. There were no significant differences in maternal and neonatal outcomes.

Key conclusions

Women’s choice on the mode of delivery and the eventual modus partus of fetuses in breech presentation is strongly influenced by the counseling technique. Vaginal breech birth in low-risk women is a safe option without long term morbidity in neonates.

Implications for practice

Counselors should be aware of their influence on women’s choice for mode of delivery in breech presentation. Counseling should be done using evidence-based information.
