Background.
Against the background of very limited evidence, the present study aimed to prospectively examine the impact of maternal postpartum PTSD symptoms on four important areas of child development, i.e. gross motor, fine motor, communication, and social-emotional development.

Methods.
This study is part of the large, population based Akershus Birth Cohort. Data from the hospital’s birth record as well as questionnaire data from 8 weeks and 2 years postpartum were used (n=1,472). The domains of child development that were significantly correlated with PTSD symptoms were entered into regression analyses. Interaction analyses were run to test whether the influence of postpartum PTSD symptoms on child development was moderated by child sex or infant temperament.

Results.
Postpartum PTSD symptoms had a prospective relationship with poor child social-emotional development two years later. This relationship remained significant even when adjusting for confounders such as maternal depression and anxiety or infant temperament. Both child sex and infant temperament moderated the association between maternal PTSD symptoms and child socialemotional development, i.e. with increasing maternal PTSD symptom load, boys and children with a difficult temperament showed to have comparatively higher levels of social-emotional problems.

Conclusions.
Examining four different domains of child development, we found a prospective impact of postpartum PTSD symptoms on children’s social-emotional development at two years of age. Our findings suggest that both boys and children with an early difficult temperament may be particularly susceptible to the adverse impact of postpartum PTSD symptoms. Additional studies are needed to further investigate the mechanisms at work.

Abstract (English)  :

Contexte.
Dans le contexte de preuves très limitées, la présente étude visait à examiner de manière prospective l’impact des symptômes post-partum du TSPT maternel sur quatre domaines importants du développement de l’enfant, soit la motricité globale, la motricité fine, la communication et le développement socio-affectif.

Méthodes
Cette étude fait partie de la grande cohorte de naissance d’Akershus basée sur la population. Les données du dossier de naissance de l’hôpital ainsi que les données du questionnaire de 8 semaines et de 2 ans post-partum ont été utilisées (n = 1 472). Les domaines du développement de l’enfant qui étaient significativement corrélés avec les symptômes du SSPT ont été entrés dans les analyses de régression. Des analyses d’interaction ont été effectuées pour tester si l’influence des symptômes du SSPT post-partum sur le développement de l’enfant était modérée par le sexe de l’enfant ou le tempérament du nourrisson.

Résultats.
Les symptômes du SSPT post-partum avaient une relation prospective avec le développement socio-affectif pauvre de l’enfant deux ans plus tard. Cette relation est demeurée significative même lors de l’ajustement pour les facteurs de confusion tels que la dépression maternelle et l’anxiété ou le tempérament du nourrisson. Le sexe des enfants et le tempérament du nourrisson ont modéré l’association entre les symptômes maternels de stress post-traumatique et le développement émotionnel social des enfants, avec une charge de symptômes de stress post-traumatique plus élevée chez les garçons et les enfants ayant un tempérament difficile.

Conclusions
En examinant quatre domaines différents du développement de l’enfant, nous avons trouvé un impact potentiel des symptômes du SSPT post-partum sur le développement socio-affectif des enfants à l’âge de deux ans. Nos résultats suggèrent que les garçons et les enfants ayant un tempérament difficile précoce peuvent être particulièrement sensibles à l’impact négatif des symptômes du SSPT post-partum. Des études supplémentaires sont nécessaires pour approfondir les mécanismes à l’œuvre.
